A CT scan showed facial trauma and thoracic trauma with dislocation of the costo-sternal junction of the right first rib and fracture of the costovertebral junction of the 1st and 2nd ribs. He was admitted to the Anaesthesia Intensive Care Unit. He was put on mechanical ventilation. On the tenth day of admission, chest movements triggered by the patient's cough were observed, with elevation of the right hemithorax observed with haemodynamic repercussions (bradycardia less than 30 beats per minute and severe arterial hypotension). A CT scan was requested, which showed a herniation in the right anterior chest wall with pleuro-pulmonary protrusion and associated soft tissue haematoma (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). Surgical repair was performed on the twelfth day of admission.
