Resumen

El cáncer colorrectal es el tercer tumor con mayor incidencia en la población mundial y supone la segunda causa de muerte según el estudio Globocan. El CDX2 ha ido adquiriendo un papel importante como marcador sensible y específico en el diagnóstico del cáncer colorrectal. Sin embargo, la ausencia de inclusión de este marcador en las guías de anatomía patológica junto con la falta de estudios existentes impide que se use en el día a día. Pese a que múltiples estudios relacionan su ausencia de tinción con un peor pronóstico, la literatura no delimita cuán intensa debe ser la tinción para considerarse positiva o negativa. En el presente estudio, se describe el H-Score como método para determinar la positividad de tinción del CDX2, mediante un software de libre acceso llamado QuPath con una muestra de 169 pacientes. Además, se sugiere que aquellos pacientes cuyos tumores presentaron un H-Score para CDX2 menor o igual a 152 puntos tuvieron un tiempo libre de recidiva significativamente inferior a aquellos con un H-Score superior a este umbral. Es por ello, que mediante este estudio se pretende resaltar la importancia que tiene la cuantificación empleando la patología digital, como podría aplicarse en la práctica diaria y se sugiere un valor de referencia para CDX2 a partir del cual el pronóstico tumoral puede diferir.

Abstract

Colorectal cancer is the third tumor with the highest incidence in the world population and is the second cause of death according to the Globocan study. CDX2 has been acquiring an important role as a sensitive and specific marker in the diagnosis of colorectal cancer. However, the lack of inclusion of this marker in the pathology guidelines together with the lack of existing studies prevent its daily use. Although multiple studies relate the absence of staining to a worse prognosis, the literature does not define how intense the staining must be to be considered positive or negative. In the present study, the H-Score is described as a method to determine the positivity of CDX2 staining, using free access software called QuPath with a sample of 169 patients. Furthermore, it is suggested that those patients whose tumors had an H-Score for CDX2 less than or equal to 152 points had a significantly shorter recurrence-free interval time compared to those with an H-Score greater than this threshold. For this reason, this study aims to highlight the importance of quantification using digital pathology, as it could be applied in daily practice, and suggests a reference value for CDX2 from which the tumor prognosis may differ.
