A 60-year-old male underwent total thyroidectomy with central and right lateral lymph node dissection in 2006 for medullary carcinoma. In 2013, he had surgery for cervical recurrence. During follow-up in 2019, CT scan revealed a metastatic cervical lymphadenopathy in sector II (Fig. 1).
In the oncology committee, we decided to perform radio-guided surgery by implanting an I125 capsule to confirm its position. After excision, the specimen was studied by x-ray (Fig. 2), and pathology results confirmed the metastasis.
The use of I125 capsules in recurrent lesions is effective because of its simple placement and greater precision in intraoperative localization. Its use in recurrent medullary thyroid cancer had not been described to date.
