El nódulo y/o masa cervical constituye un motivo frecuente de consulta tanto en Atención Primaria como en las urgencias hospitalarias. Es necesario tener claro el manejo de estas situaciones, lo cual implica una anamnesis metódica, una exploración sistemática y la elección de las pruebas complementarias adecuadas. La historia clínica ha de incluir numerosos aspectos entre los que no deben faltar nunca edad y duración de la masa. La exploración física es la exploración básica otorrinolaringológíca: fosas nasales, orofaringe, hipofaringe, laringe y exploración del cuello y cuero cabelludo. Si la masa no cumple criterios iniciales de derivación, probablemente tenga un origen inflamatorio/infeccioso, requerirá una actitud expectante y tratamiento sintomático, así como tratamiento antibiótico empírico en los cuadros en los que se sospeche infección no vírica.
The nodule and/or cervical mass constitute a frequent reason for a visit to the doctor both in Primary Health Care as well as in the hospital emergency service. Management of these situations should be clear. This implies a methodical anamnesis, systematic examination and choice of adequate complementary tests. The clinical history must include many aspects, that should always include age and duration of the mass. The physical examination is the basic ENT examination: nasal pits, oropharynx, hypopharynx, larynx and examination of neck and scalp. If the mass does not fulfill the initial criteria of referral, it probably has an inflammatory/ infectious origin. It will require a waiting attitude and symptomatic treatment, as well as empiric antibiotic treatment in the pictures in which non-viral infection is suspected.
