Patients are increasingly being treated with these drugs (1% of the Spanish population in 2021<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>), and if any of these patients need to undergo surgery or an invasive procedure, their DOAC therapy may need to be interrupted.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Although different scientific societies have established safe interruption windows based on pharmacokinetic data for both scheduled surgery<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> and regional anaesthesia-analgesia techniques,<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> in order to maximise safety, plasma levels of DOACs should be monitored in patients with impaired kidney function; particularly when performing loco-regional techniques.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Following such specific recommendations can be problematic in clinical practice if clinicians are unsure of the type of test that should be performed or when testing is unavailable. We performed a survey to determine the real availability of specific DOAC monitoring tests in Spanish hospitals and the extent to which anaesthesiologists understand and use these tests. The Spanish Society of Anaesthesiology, Resuscitation and Pain Therapy (SEDAR) sent the survey questionnaire to all its members and also posted it on social media networks (Supplementary documentation 1 – Form).</p><p id="par0020" class="elsevierStylePara elsevierViewall">After a collection period of 5 months (March–July 2022), 152 responses from 100 hospitals were received. Clinicians were unaware that tests for specific DOACs were available (48% in the case of dabigatran and 31% in the case of anti-Xa) and in many centres these tests were unavailable (60% and 49% for dabigatran and anti-XA, respectively) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). In centres that were able to perform specific tests, they were mostly ordered in patients with acute bleeding or undergoing urgent surgery (31% in both cases), and were ordered per protocol only in 1% of cases.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">This is the first survey performed to determine the extent to which anaesthesiologists are aware of these tests and their availability in each hospital, and shows that many anaesthesiologists are unaware of the possibility of monitoring DOAC levels; particularly dabigatran and less so anti-Xa. The unavailability of testing is another striking finding, particularly when this information is combined with the time frame in which it is available: round the clock in only 8% and 18% of centres for dabigatran and anti-Xa, respectively.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The first conclusion to be drawn from this survey is probably that training in such an important issue as DOAC monitoring (which is obviously deficient) needs to be stepped up: most anaesthesiologists do not follow existing recommendations because they are unaware of the existence of DOAC tests.</p><p id="par0035" class="elsevierStylePara elsevierViewall">This article describes the results of a survey sent from the Scientific Society (SEDAR). Surveys are frequently used in scientific research to rapidly obtain large data sets, and interpreting the results give researchers indirect insight into certain issues. Their value lies more in their target population group than in the opinion of individual respondents, and this is why SEDAR was informed of the aim of the survey, and agreed to send the questionnaire to its affiliates and post it on social networks. Therefore, although the result might not be fully representative because response was obviously voluntary, it does furnish us with information on an issue that has hitherto received little attention, and allows us to address the subject of improving understanding and monitoring of DOACs in a large sample of hospitals (28% of national hospitals with more than 300 beds).</p><p id="par0040" class="elsevierStylePara elsevierViewall">Despite its limitations, the results of this survey show the need to improve training and dissemination of DOAC monitoring in hospitals in general, and specifically in the perioperative period of patients with kidney failure.
