Las fracturas de cadera, muy frecuentes en el anciano, constituyen un grave problema de salud debido a su alta y creciente frecuencia. Después de recordar su etiopatogenia, clasificación y diagnóstico, se hace un repaso a los distintos métodos terapéuticos disponibles: ortopédicos y quirúrgicos. Entre estos últimos se distinguen los métodos de osteosíntesis extramedulares e intramedulares, y se comparan según la Medicina Basada en la Evidencia. El éxito del tratamiento de las fracturas de la región trocantérea del fémur depende de la estabilidad de la osteosíntesis practicada. En la actualidad, los métodos de osteosíntesis preferidos son el tornillo-placa deslizante y el clavo gamma, aunque tampoco faltan los servicios donde se prefieren los clavos de Ender u otros sistemas clavo-placa. Sin abundar en los motivos, lo más lógico parece ser considerar cada caso de manera individual, sin renunciar a ningún método de síntesis de todos los disponibles y, quizá, concediendo más protagonismo a las artroplastias de sustitución.

Hip fractures are common in the elderly and are a serious health problem due to their high and growing frequency. After recording the etiopathogenesis, classification, and diagnosis, a review was made of nonoperative and surgical therapeutic models. These models include extramedullary and intramedullary osteosynthesis methods, which were compared using criteria of evidence-based medicine. The success of treatment of fractures of the trochanteric region of the femur depends on the stability of the bone fixation. At present, the preferred osteosynthesis methods are the sliding screw-plate and Gamma nail, although Ender nails and other nail-plate systems are preferred for no special reason in other services. It would seem logical that each case be considered individually, without renouncing any synthesis method among those available. It is suggested that more importance should be given to hip prostheses.
