An unclear association" ] ] "textoCompleto" => "<p class="elsevierStylePara"><span class="elsevierStyleBold">INTRODUCCIÓN</span></p><p class="elsevierStylePara">Durante años, los poetas y escritores, así como el folclore popular, han hablado de la relación entre nuestro estado de ánimo y nuestro corazón; sin embargo, esta relación no se ha evaluado científicamente hasta hace unas pocas décadas<span class="elsevierStyleSup">1</span>, lo que resulta extraño si consideramos que las enfermedades mentales y del corazón son muy prevalentes en la sociedad actual, deterioran la calidad de vida de las personas que las padecen, suponen una carga económica enorme y existe una evidencia cada vez más consistente que las vincula<span class="elsevierStyleSup">2</span>.</p><p class="elsevierStylePara">La cardiopatía isquémica (CI) es la primera causa de morbimortalidad en los países desarrollados<span class="elsevierStyleSup">3</span>; por su parte, la patología afectiva constituye el principal motivo de consulta psiquiátrica en atención primaria con un carácter claramente emergente<span class="elsevierStyleSup">4</span>.</p><p class="elsevierStylePara">Los sujetos con síntomas depresivos tienden a presentar un riesgo superior de ataques y complicaciones de tipo cardíaco que serían responsables del incremento de la mortalidad asociada a la enfermedad depresiva, lo que también se ha observado en otros enfermos mentales<span class="elsevierStyleSup">5</span>. Los datos de tipo prospectivo obtenidos en la población sana han demostrado el papel de los síntomas depresivos en el desarrollo y el pronóstico de la enfermedad coronaria: el riesgo de CI en pacientes con depresión es un 60% mayor, con un riesgo relativo de 1,64<span class="elsevierStyleSup">6,7</span>. Se ha señalado un incremento de la mortalidad hasta 2 veces superior en pacientes depresivos<span class="elsevierStyleSup">3</span>, que sería independiente de otros factores de riesgo clásicos<span class="elsevierStyleSup">8</span> y no dependería de la gravedad inicial de la enfermedad cardiovascular<span class="elsevierStyleSup">1</span>. La gravedad del cuadro depresivo se ha relacionado con el riesgo de mortalidad cardíaca: 1,6 para las depresiones menores y 3 para las depresiones mayores<span class="elsevierStyleSup">2,9</span>. Este mal pronóstico se mantendría en el tiempo<span class="elsevierStyleSup">2</span>.</p><p class="elsevierStylePara">Los síntomas depresivos de mayor duración parece que tienen un menor impacto fisiológico, quizás porque se ha tenido más tiempo para adaptarse al estrés emocional, con un mayor ajuste de las expectativas y una valoración de la experiencia previa<span class="elsevierStyleSup">10</span>.</p><p class="elsevierStylePara">A pesar de que en los estudios epidemiológicos sobre la población general se observan diferencias en la incidencia de depresión entre varones y mujeres, no se han constatado estas diferencias en el riesgo de mortalidad en pacientes con cardiopatía y depresión. Sin embargo, las mujeres presentaban una sintomatología más genuinamente depresiva mientras los varones presentaban más síntomas de tipo ansioso<span class="elsevierStyleSup">11</span>.</p><p class="elsevierStylePara">Penninx et al<span class="elsevierStyleSup">12</span> observaron que la depresión (sobre todo las formas más graves) incrementaba el riesgo de mortalidad en sujetos con y sin enfermedad cardíaca previa, lo que podría justificar una agresiva detección y un tratamiento del cuadro psiquiátrico en pacientes cardiópatas, tanto a nivel primario como secundario. Sin embargo, los datos reales no permiten confirmar una relación de tipo causal, y este tipo de intervenciones serían cuanto menos prematuras<span class="elsevierStyleSup">13</span>.</p><p class="elsevierStylePara">Los estudios mencionados demuestran una relación de riesgo significativa en lo que parece ser algo más que una respuesta emocional secundaria a la enfermedad cardíaca, aunque en ningún caso se ha establecido la posible causalidad entre ambas enfermedades<span class="elsevierStyleSup">14</span>.</p><p class="elsevierStylePara">Algunos autores plantean que los pacientes cardíacos tienden a "normalizar" su depresión, atribuyen síntomas somáticos frecuentes en la enfermedad depresiva a su enfermedad cardíaca y escasamente consideran un origen emocional para éstos<span class="elsevierStyleSup">6</span>. Los síntomas depresivos suelen ser menos directos y a menudo son reemplazados por otros menos típicos: cansancio inusual, falta de energía y síntomas físicos no justificados por la exploración (disnea, palpitaciones o dolor precordial...). Algunos clínicos utilizan el término agotamiento vital para describir el estado afectivo negativo, en el que el cansancio es la queja fundamental de estos pacientes<span class="elsevierStyleSup">15</span>.</p><p class="elsevierStylePara">Hay una tendencia a infradiagnosticar la patología afectiva en estos pacientes y, por tanto, a tratarlos de una manera inadecuada. Habitualmente, se considera normal que haya algún grado de tristeza como respuesta emocional humana ante una situación que supone sufrimiento físico y riesgo vital, esto junto a la peculiaridad clínica dificulta nuestra capacidad para detectar los cuadros patológicos. A esta situación cabe añadir el estigma social de la enfermedad mental, que retrae a los pacientes, los familiares e incluso a nuestros colegas médicos, a la hora de solicitar una colaboración con los servicios de salud mental<span class="elsevierStyleSup">15</span>.</p><p class="elsevierStylePara">No existe todavía consenso sobre los mecanismos que vinculan la depresión y la CI. Se han propuesto una serie de factores de naturaleza biológica y otros de tipo psicológico o conductual. Entre estos últimos citamos la asociación entre la depresión y las conductas poco saludables para el corazón (tabaquismo, baja actividad física, factores dietéticos y escaso cumplimiento de las recomendaciones farmacológicas), las dificultades de afrontamiento, el aislamiento y estrés vital crónico. Se recogen de Musselman<span class="elsevierStyleSup">14</span> y Appels<span class="elsevierStyleSup">6</span> los fenómenos fisiopatológicos que se proponen como nexo de unión de ambas patologías (factores de tipo autoinmune e inflamatorio, eje hipotálamo-hipofisario-suprarrenal, trastornos cardíacos asociados al estrés crónico, factores plaquetarios...).</p><p class="elsevierStylePara">El tratamiento de los cuadros afectivos posee un interés no sólo sanitario sino también como prueba científica de valor innegable, porque la demostración de que una reducción de la depresión induce una disminución del riesgo en la morbimortalidad cardíaca implicaría considerar la depresión como un factor de riesgo para la CI. El estudio ENRICH (Enhacing Recovery in Coronary Heart Disease) fue diseñado para determinar si el tratamiento de los síntomas depresivos conseguía reducir el riesgo de reinfarto y de muerte en estos pacientes. En un primer análisis no se encontraron diferencias entre los pacientes a quienes se realizan cuidados usuales y los incorporados a la intervención específica para tratar el ánimo depresivo<span class="elsevierStyleSup">16</span>. Un segundo análisis de este estudio se centró en un subgrupo de pacientes de mayor gravedad clínica, y se indica que los que no mejoraron en su estado de ánimo, a pesar de una intervención terapéutica intensiva, tenían una mayor mortalidad a largo plazo. Esta situación plantea la cuestión de la dificultad en la asignación etiología de los síntomas. Los autores creen que puede haber un tipo de depresión que confiere una mayor mortalidad cardiovascular, que se podría incluir entre las denominadas depresiones vasculares de inicio tardío, compartiendo alteraciones fisiopatológicas y una mala respuesta a las terapias estándares. En estos momentos, la diferenciación entre la depresión vascular y no vascular es compleja y la realización sistemática de resonancia magnética (RM) y tests neuropsicológicos resulta excesiva, por lo que debe restringirse a los pacientes de alto riesgo<span class="elsevierStyleSup">17</span>.</p><p class="elsevierStylePara">Creemos, junto con Glassman y Shapiro<span class="elsevierStyleSup">1</span>, que la depresión puede asociarse con la CI, y es un factor que influye negativamente no sólo sobre la calidad de vida de nuestros pacientes, sino también sobre la evolución de la patología cardíaca.</p><p class="elsevierStylePara"><span class="elsevierStyleBold">OBJETIVOS</span></p><p class="elsevierStylePara">Se plantea como objetivo de este estudio detectar la sintomatología depresiva en los pacientes hospitalizados por CI. Como hipótesis principal de trabajo consideramos que los pacientes ingresados por CI tienen más posibilidades de presentar depresión, e investigamos la relación de esta variable principal con otros factores de tipo sociodemográfico, factores de riesgo clásicos, antecedentes de enfermedad somática y psiquiátrica.</p><p class="elsevierStylePara"><span class="elsevierStyleBold">MATERIAL Y MÉTODO</span></p><p class="elsevierStylePara">Se diseñó un estudio descriptivo observacional, de tipo prospectivo y abierto. La investigación se realizó en el Hospital Royo Villanova, perteneciente al sector I de Zaragoza (que incluye 178.000 habitantes), con una capacidad de 260 camas y una cartera de servicios propia de un hospital comarcal. Se recogieron datos de todos los pacientes ingresados en el servicio de cardiología por CI, desde el 1 de mayo de 2003 hasta el 30 de abril de 2004. Con tal fin se diseñó un sistema de alerta de posibles casos en el que se coordinaron los distintos servicios del hospital. De esta manera se obtuvieron 96 casos de CI de un total de 637 pacientes ingresados en el servicio durante ese período. De los 96 casos, 4 decidieron no colaborar en el estudio y otros 6 fueron descartados por diferentes problemas documentales y de seguimiento, quedando la muestra constituida por 86 pacientes, cuyas principales características se detallan en la tabla 1.</p><p class="elsevierStylePara"><img src="46v13n03-13090530tab01.gif"></img></p><p class="elsevierStylePara">A propósito del estudio se diseñó una ficha de recogida de datos: de filiación y sociodemográficos, antecedentes patológicos (somáticos y psiquiátricos) y relacionados con la hospitalización. La recogida de datos se intentó que fuera lo más fiable y extensa posible, acudiendo a diversas fuentes (paciente, familia, historia clínica y cardiólogo responsable).</p><p class="elsevierStylePara">Se informó y se solicitó autorización a la dirección del hospital y a la comisión deontológica. A todas las personas que decidieron participar se les solicitó el consentimiento informado para participar en el trabajo.</p><p class="elsevierStylePara">A todos los pacientes incluidos en la muestra se les faci litó el cuestionario autoadministrado (GHQ), versión de 28 ítems<span class="elsevierStyleSup">18,19</span>, se permitió la ayuda de la familia en caso de tener dificultades para leerlo y se planteó como punto de corte de posible caso psiquiátrico una puntuación de 7 (se revisaron cuidadosamente cada uno de los cuestionarios, de tal manera que se descartaron errores en la puntuación, o sesgos manifiestos sobre la escala somática).</p><p class="elsevierStylePara">A los pacientes que tuvieron un GHQ positivo se les realizó una entrevista clínica diagnóstica y se administró la escala de Hamilton para la depresión, que establece una medida del estado depresivo y aporta una idea sobre la significación clínica de los síntomas depresivos<span class="elsevierStyleSup">20,21</span>. Se eligió la versión de 17 ítems con un punto de corte para la depresión en una puntuación de 10.</p><p class="elsevierStylePara">El tratamiento estadístico se efectúo mediante el paquete informatizado SPSS 12.0 (versión española). Para el contraste de hipótesis en el análisis estadístico se consideró un nivel de significación con un valor de p < 0,05 para rechazar la hipótesis nula. Se emplearon tests paramétricos siempre que la distribución de la muestra fuera normal y permitiera su utilización.</p><p class="elsevierStylePara"><span class="elsevierStyleBold">RESULTADOS</span></p><p class="elsevierStylePara">La muestra estaba formada por sujetos no ancianos, con más frecuencia varones, con un nivel académico básico y que residieran con sus familias (tabla 1).</p><p class="elsevierStylePara">La mayoría de los pacientes (90%) tenía algún tipo de antecedente médico, sobre todo cardíaco, y es muy frecuente la combinación de varias enfermedades en más de 3 sistemas orgánicos (tabla 2). Un 32% de los pacientes ya había ingresado previamente por problemas cardíacos. Los factores de riesgo clásicos en la CI se recogen en más del 90% de los pacientes y, con gran frecuencia, se combinan varios de ellos; en un 54,7% están presentes dos o más (p. ej., hipertensión arterial [HTA] con tabaquismo y diabetes). El factor de riesgo clásico que aparece de manera aislada con más frecuencia es la HTA (tabla 3).</p><p class="elsevierStylePara"><img src="46v13n03-13090530tab02.gif"></img></p><p class="elsevierStylePara"><img src="46v13n03-13090530tab03.gif"></img></p><p class="elsevierStylePara">Los antecedentes psiquiátricos están presentes en más de una cuarta parte de los casos, y los trastornos de ansiedad ocupan el primer lugar (8,1%); obviamente, los ansiolíticos fueron los fármacos más utilizados (tabla 2).</p><p class="elsevierStylePara">El cuestionario GHQ fue positivo en 33 pacientes (38,4%) (tabla 3) y se apreció depresión con una escala de Hamilton positiva en 10 de ellos (un 33% de los pacientes con GHQ positivo), que equivale al 11,6% de la muestra total (tabla 3). En el momento del alta hospitalaria, a 9 pacientes (10,5%) se les prescribió un tratamiento psicofarmacológico, sobre todo antidepresivos (tabla 3). Al alta, los pacientes recibieron una media de 2 diagnósticos cardiológicos, y el número medio de fármacos fue de 5 (rango, 0-13), no incluidos los psicofármacos.</p><p class="elsevierStylePara">En el análisis estadístico se alcanzó significación estadística entre la escala GHQ y las siguientes variables: edad, antecedentes personales de tipo psiquiátrico (de manera global, sin asociarse significativamente con ningún diagnóstico) y tratamiento con psicofármacos al alta. No se observaron relaciones significativas con otros datos de filiación, ni con las variables de enfermedad somática. La segunda variable dependiente investigada fue la detección de depresión y escala de Hamilton, que se asoció de una manera significativa con antecedentes personales de depresión (no con otros tipos de trastorno), ingresos psiquiátricos previos, antecedentes familiares de enfermedad psiquiátrica y tratamiento con antidepresivos al alta (tabla 4).</p><p class="elsevierStylePara"><img src="46v13n03-13090530tab04.gif"></img></p><p class="elsevierStylePara"><span class="elsevierStyleBold">DISCUSIÓN</span></p><p class="elsevierStylePara">El objetivo fundamental del estudio fue evaluar el grado de malestar psíquico en pacientes hospitalizados por CI. Para ello, se empleó en una primera fase el GHQ, que sirvió de instrumento de cribado para el empleo posterior de una entrevista clínica, y una escala que permitieran diagnosticar la depresión en los pacientes que presentaron mayor malestar. Barth<span class="elsevierStyleSup">3</span> investiga la utilidad en la detección de sintomatología depresiva de los métodos de autoevaluación y de las entrevistas clínicas estandarizadas, sin encontrar diferencias definitivas entre ambos métodos. Lesperance y Frasure Smith<span class="elsevierStyleSup">15</span> señalan que un cribado sistemático de la depresión en pacientes cardiópatas sólo está justificado cuando se dispone de suficientes medios para confirmar el diagnóstico y cuando se puede proveer un adecuado tratamiento.</p><p class="elsevierStylePara">Se ha investigado una muestra de pacientes caracterizados por presentar patología somática a su ingreso (cardiopatía) y por sus antecedentes somáticos, con una presencia abrumadora de los factores de riesgo clásicos en esta patología (hipertensión, tabaquismo, dislipemias y diabetes). Por tanto, se pretendió investigar si estos pacientes presentan más depresión, y si esto se relaciona con la patología somática (fundamentalmente con la gravedad). En una segunda fase del estudio, actualmente en fase de recogida de datos, se pretende investigar si los pacientes que padecieron una depresión durante la hospitalización tuvieron una mayor morbimortalidad y, por tanto, si podría considerarse como un factor de riesgo sobreañadido en la CI<span class="elsevierStyleSup">2,3,8,9</span>.</p><p class="elsevierStylePara">En casi la mitad de los pacientes se ha detectado un malestar psíquico, tal como se mide con el GHQ, obviamente mayor al obtenido en otras poblaciones no clínicas<span class="elsevierStyleSup">22</span> y dentro de las cifras que se obtienen en pacientes hospitalizados (entre un 12 y 55%)<span class="elsevierStyleSup">24</span>. El dato no resulta sorprendente y por sí solo no tiene más interés que realizar la selección de los pacientes. No obstante, podemos destacar algunas relaciones que se obtuvieron en el análisis posterior. A priori sorprende que los pacientes de mayor edad tengan menor malestar, cuando son los que presentan mayor deterioro orgánico y funcional; consideramos que los factores de tipo psicológico (respuestas de tipo adaptativo y de temor) van a ser determinantes en el malestar de los más jóvenes. La influencia de los factores psicológicos se ve avalada al observar una asociación significativa entre las puntuaciones en el GHQ y los antecedentes psiquiátricos, pero no con variables asociadas a la cardiopatía ni a la enfermedad física.</p><p class="elsevierStylePara">Un elevado porcentaje de los pacientes que se situaron por encima del punto de corte en el GHQ presentaban un cuadro depresivo (más de la tercera parte), que representa un 11,6% del total de la muestra investigada. Estas cifras son más elevadas que las de la población general (un 2-5%)<span class="elsevierStyleSup">24</span>, y no se han hallado diferencias entre mujeres y varones; también son más bajas que las recogidas en la bibliografía, que oscilan entre un 16 y un 23%<span class="elsevierStyleSup">14</span>.</p><p class="elsevierStylePara">Como es razonable, el número de pacientes tratados con antidepresivos se correspondió con la patología detectada, modificándose la pauta de prescripción que se había encontrado en los antecedentes, con lo que disminuyó el porcentaje de ansiolíticos empleados. Los pacientes con enfermedad cardiovascular y depresión deben ser objeto de una intervención que incluya algún tipo de psicoterapia y psicofármacos, todo ello integrado en la rehabilitación cardíaca<span class="elsevierStyleSup">14</span>. En cuanto a las posibilidades farmacológicas, la investigación sobre la eficacia, la seguridad y la tolerabilidad del tratamiento para la depresión en pacientes con una enfermedad física es todavía bastante limitada. Los antidepresivos tricíclicos parecen conllevar un mayor riesgo en estos pacientes por la hipotensión postural que pueden inducir, por tener un cierto efecto seudoquinidínico y por las frecuentes (y potencialmente graves) interacciones con otros fármacos somáticos, indicados con frecuencia en estos pacientes. Las actuales indicaciones de este tipo de antidepresivos en pacientes cardiópatas serían: depresiones resistentes a, al menos, 2 de los antidepresivos de nueva generación y en episodios graves con antecedente de respuesta previa y con bajo riesgo de arritmia ventricular. Los nuevos antidepresivos parecen tener la misma eficacia y una seguridad superior (escasos efectos cardiovaculares y sobredosis). El único efecto conocido sobre el corazón de los inhibidores selectivos de la recaptación de serotonina es un grave enlentecimiento del seno, muy poco frecuente. Se han descrito algunas alteraciones en la homeostasia y la agregación plaquetaria con fluoxetina, aunque han demostrado ser fármacos seguros y con cierta eficacia<span class="elsevierStyleSup">25</span>. Las interacciones farmacológicas, la mayoría inofensivas, nos obligan a tener precaución en algunos casos: los antidepresivos que inhiben el citocromo P4502D6 (paroxetina, fluoxetina y probablemente sertralina) no deberían combinarse con antiarrítmicos, como encainamida, flecainida, mexileteno o propafenona; además, pueden incrementar los valores de los bloqueadores beta. La nefazadona, la fluvoxamina y la fluoxetina inhiben el citocromo P4503A3/4 responsable del metabolismo de las medicaciones utilizadas con frecuencia en estos pacientes: estatinas, bloqueadores del calcio, ciclosporina y algunos antiarrítmicos. Algunos antihistamínicos (astemizol, terfenadina y cisapride) también son contraindicados con nefazadona, fluoxetina, y fluvoxamina<span class="elsevierStyleSup">14,15</span>. Se alerta sobre la hierba de San Juan por las interacciones frecuentes que tiene con otras sustancias cardiotónicas<span class="elsevierStyleSup">12</span>.</p><p class="elsevierStylePara">En pacientes con depresión leve-moderada, o en los que no pueden beneficiarse del tratamiento farmacológico, la psicoterapia de corta duración y bien estruc-turada parece ser un excelente recurso terapéutico de primera línea. La terapia cognitivo-conductual y la psicoterapia interpersonal han mostrado una eficacia reconocida en 12-14 sesiones. Se recomienda este abordaje también cuando hay un elevado riesgo de recurrencia o de recaída<span class="elsevierStyleSup">15</span>.</p><p class="elsevierStylePara">En este trabajo la depresión como variable de estudio resultó independiente de todos los indicadores de enfermedad somática que se analizaron. Resultaría difícil identificar pacientes depresivos a partir de sus antecedentes somáticos (cardiopatía, factores de riesgo, hospitalizaciones previas, etc.). La gravedad de la CI medida por el número de diagnósticos de tipo cardíaco y número de fármacos (no psiquiátricos) no se asoció con la presencia de depresión. Por el contrario, los indicadores de enfermedad psiquiátrica previa sí estuvieron presentes en los pacientes depresivos: antecedentes previos de depresión, tratamiento con antidepresivos, antecedentes familiares y de ingreso.</p><p class="elsevierStylePara">Consideramos que los datos no aportan evidencia suficiente que relacione de una manera directa la patología afectiva con la CI, tal como han sugerido diversos autores recientemente<span class="elsevierStyleSup">26</span>. Se detecta un importante malestar psíquico que no está originado por la depresión más que en una tercera parte de los casos. Sospechamos que el malestar emocional, en la mayoría de los casos, obedece a causas relacionadas con la propia hospitalización: malestar somático, temores, desubicación, ambiente extraño, etc. Por el contrario, el perfil depresivo se correlaciona con los factores clásicos en esta patología: antecedentes personales y familiares con algún trastorno depresivo.</p><p class="elsevierStylePara">En la actualidad, no se reconoce ningún factor psicosocial de riesgo mayor en la enfermedad coronaria, por muy evidente que sea la relación entre ambas enfermedades<span class="elsevierStyleSup">7</span>. Para considerar la depresión como un factor de riesgo independiente en la CI, hace falta algo más que una asociación epidemiológica; es necesario caracterizar, conocer e identificar los mecanismos biológicos que relacionen ambas enfermedades, algo todavía lejos de conseguir, y también es necesario demostrar que el tratamiento adecuado de esta patología, o su prevención, sea capaz de disminuir la morbimortalidad de los pacientes con CI<span class="elsevierStyleSup">27-29</span>. En los estudios más recientes hemos percibido que la fuerza de la asociación entre depresión y CI ha disminuido, quizás porque se publican más estudios con resultados negativos, o porque se tratan mejor los mecanismos fisiólogos y conductuales subyacentes en la relación entre ambas enfermedades<span class="elsevierStyleSup">2</span>.</p><p class="elsevierStylePara">Reconocemos varias limitaciones en el estudio. Se trata de un estudio abierto, de campo, sin selección previa de la muestra salvo el criterio de presentar una CI, con un tamaño muestral pequeño; la información sobre antecedentes previos era la obtenida de los historiales médicos o la referida por pacientes y familiares. Elegimos como criterio de gravedad de enfermedad cardíaca el número de diagnósticos y los tratamientos empleados al alta, siendo conscientes de que esta correspondencia puede ser cuestionable en algunos pacientes.</p><p class="elsevierStylePara">Posiblemente, se ha podido infradiagnosticar la sintomatología depresiva, que en estos pacientes puede estar representada por síntomas menos típicos. Algunos autores, como Glassman y Shapiro<span class="elsevierStyleSup">1</span> y, recientemente, Frasurer<span class="elsevierStyleSup">26</span>, investigan la contribución específica de determinados componentes del síndrome psicológico en el incremento del riesgo de mortalidad, y señalan como más determinantes la inhibición social, la falta de energía y la afectividad negativa. Estos síntomas podrían ayudar a identificar de una manera más precisa la depresión en estos pacientes y facilitar la comprensión de los mecanismos fisiopatológicos subyacentes en ambas patologías (disregulación del eje hipotalámo-hipofisario, alteraciones del cortisol, neurovegetativas, plaquetarias, etc.)<span class="elsevierStyleSup">30</span>. Material y método: Se investigaron todos los pacientes hospitalizados por cardiopatía isquémica desde el 1 de mayo de 2003 al 30 de abril de 2004, quedando la muestra constituida por 86 pacientes. Se utilizó el cuestionario GHQ, versión española de 28 ítems (Lobo et al, 1986) y la escala Hamilton para la depresión (Hamilton, 1967). Resultados: El cuestionario GHQ fue positivo en un 38,4% de los pacientes. Se apreció depresión en un 33% de los pacientes con GHQ positivo, que equivale al 11,6% de la muestra total. En el momento del alta del hospital, a 9 pacientes (10,5%) se les prescribió tratamiento psicofarmacológico (fundamentalmente antidepresivos). En el análisis estadístico se alcanzó significación estadística entre la escala GHQ y los antecedentes personales psiquiátricos, en general, y entre la depresión y los antecedentes depresivos. Contexto: En un elevado porcentaje de pacientes con cardiopatía isquémica se puede apreciar algún grado de depresión, y esta asociación parece incrementar el riesgo de morbilidad y mortalidad. Además de las consecuencias sobre el pronóstico y la calidad de vida, hay dificultades para el diagnóstico y el tratamiento de la depresión en estos pacientes. Objetivo: Detectar la sintomatología depresiva en pacientes hospitalizados por cardiopatía isquémica. Material y método: Se investigaron todos los pacientes hospitalizados por cardiopatía isquémica desde el 1 de mayo de 2003 al 30 de abril de 2004, quedando la muestra constituida por 86 pacientes. Se utilizó el cuestionario GHQ, versión española de 28 ítems (Lobo et al, 1986) y la escala Hamilton para la depresión (Hamilton, 1967). Resultados: El cuestionario GHQ fue positivo en un 38,4% de los pacientes. Se apreció depresión en un 33% de los pacientes con GHQ positivo, que equivale al 11,6% de la muestra total. En el momento del alta del hospital, a 9 pacientes (10,5%) se les prescribió tratamiento psicofarmacológico (fundamentalmente antidepresivos). En el análisis estadístico se alcanzó significación estadística entre la escala GHQ y los antecedentes personales psiquiátricos, en general, y entre la depresión y los antecedentes depresivos. Conclusiones: los pacientes ingresados por cardiopatía isquémica tienen más posibilidades de sufrir malestar psíquico y depresión, incrementándose el uso de antidepresivos. La depresión está más vinculada a la patología psiquiátrica previa y no a la enfermedad cardíaca.

Background: A high percentage of patients with ischemic heart disease have some degree of depression and this association seems to increase the risk of morbidity and mortality. In addition to the repercussions on prognosis and quality of life, the diagnosis and management of depression in these patients can pose difficulties. Objective: To detect depressive symptomatology in patients hospitalized for ischemic heart disease. Material and method: All patients admitted to our hospital for ischemic heart disease from May 1, 2003 to April 30, 2004 (n = 86) were studied. The Spanish-validated version of the 28-item General Health Questionnaire (GHQ; Lobo et al, 1986) and the Hamilton scale for depression (Hamilton, 1967) were used. Results: The GHQ was positive in 38.4% of the patients. Depression was found in 33% of the patients with a positive GHQ, representing 11.6% of the whole sample. At discharge, nine patients (10.5%) were prescribed psychopharmacological treatment (mainly antidepressants). Statistically significant associations were found between GHQ score and unspecified personal psychiatric antecedents on the one hand and between depression and a prior history of depression on the other. Conclusions: Patients hospitalized for ischemic heart disease have an increased likelihood of suffering from psychic discomfort and depression and consequently the use of antidepressants has increased. Depression is more closely linked to prior psychiatric disorders than to heart disease.
