The patient was haemodynamically stable, intubated (radiopaque, PVC 7.5 endotracheal tube with high volume low pressure cuff, Magill curve and Murphy eye) connected to mechanical ventilation. During the initial evaluation we checked the tube placement, presenting EtCO2 41 and decreased bilateral vesicular murmur.A cranial-cervical CT scan was performed, which showed multiple fractures of the left orbital-maxillary and malar, Hemi-Lefort III, longitudinal fracture of the left petrous bone and malposition of the endotracheal tube, with distal tip at the commissure of the glottis, and ventilation through Murphy eye (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).<elsevierMultimedia ident="fig0005"></elsevierMultimedia>The Murphy eye consists of a hole located on the side opposite to the direction of the tube bevel. It facilitates ventilation if the bevel is obstructed. In this case, the tip of the tube was impacted in the anterior commissure of the glottis, and the Murphy eye was situated between the arytenoids and the epiglottis, allowing bilateral pulmonary ventilation. The patient was transferred to the operating room and the endotracheal tube was placed under direct vision with a bronchoscope, without requiring a tracheostomy." This is a monthly journal (10 issues) that publishes scientific articles of all areas within its specialty: clinical anesthesia, resuscitation-intensive medicine and critical care, diagnosis and treatment of acute and chronic pain, urgent care and emergencies and studies on basic science and related fields.
