He was admitted to the Intensive Care Unit (ICU) due to multiple trauma with severe head injury and fractures of the tibia-fibula and humerus.</p><p id="par0010" class="elsevierStylePara elsevierViewall">After prolonged respiratory weaning, we decided to assess the feasibility of performing percutaneous tracheostomy (PT). We performed an ultrasound examination of the neck to visualize the position of the tracheal rings and rule out the existence of anatomical anomalies.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The scan showed the presence of a large calibre vessel (2−3 cm in diameter) anterior to the trachea at the level of the first and third tracheal rings (<a class="elsevierStyleCrossRefs" href="#fig0005">Figs. 1 and 2</a>).
