The estimated prevalence in Portugal in 2014 was 36.6%, higher than the European average (between 5.9% and 19.9%)<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Many guidelines recommend using a multimodal and multidisciplinary approach to CLP, consisting of (1) patient education on the importance of physical activity and regular exercise, (2) physical therapy, (3) pharmacological treatment, such as non-steroidal anti-inflammatory drugs, opioids and antidepressants, (4) psychological interventions and, if indicated, (5) invasive treatments (for example, injections or surgery)<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3</span></a>. Despite these interventions, this type of multifactorial pain is often difficult to treat, leading to frustration and anxiety among patients and a considerable impact on their quality of life<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Ultrasound-guided erector spinae plane (ESP) block is an interfascial technique in which local anaesthetic is injected between the erector spinae muscle and the transverse process of thoracic or lumbar vertebrae to block the ventral and dorsal rami of the spinal nerves. It was originally described by Forero et al.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> as a simple and safe interfascial technique to relieve thoracic neuropathic pain. Since then, it has been used to treat acute pain in many contexts, such as spinal surgery<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,6</span></a> and chronic pain<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,7</span></a>.</p><p id="par0020" class="elsevierStylePara elsevierViewall">In view of the recent scientific developments in this technique, our objective was to determine the analgesic efficacy of ultrasound-guided ESP block in a group of patients with moderate to severe CLP. We describe 10 clinical cases and their response to this analgesic approach up to 1 month after the procedure.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">We received approval from the Ethics Committee to perform the ultrasound-guided ESP block in a selected group of patients with moderate to severe CLP refractory to a wide range of systemic analgesics. All patients were undergoing follow-up in the Pain Clinic of a tertiary hospital, and were selected consecutively. Inclusion criteria were: age over 18 years, long-term CLP (musculoskeletal, neuropathic and/or surgical), paravertebral muscle spasms and stiffness, no response to pharmacological treatment, visual analogue scale (VAS) > 4, not contraindicated for ESP block, and willingness to participate in the study and sign an informed consent form. Only patients that were able to attend our pain clinic and receive treatment were included in the study.</p><p id="par0030" class="elsevierStylePara elsevierViewall">All patients received ESP block administered using an Echoplex-80 mm (Vygon®) 22 gauge needle guided by a SonoSite Edge II ultrasound system with a 2−5 MHz convex transducer. Intra-procedural monitoring consisted of ECG, non-invasive blood pressure, pulse oximetry, and previous intravenous evaluation. The blocks were performed bilaterally at the level of L1-L2. With the patient in the prone position (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>), the transducer was placed longitudinally to facilitate identification of the transverse processes at the target level (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). Using an in-plane technique, the needle was advanced in a cranio-caudal direction until it contacted the bone (transverse process). After injecting a small dose of local anaesthetic (LA) to confirm correct needle placement, 0.2% ropivacaine (20 ml) and dexamethasone (4 mg) were injected bilaterally in all patients except one, who received unilateral right-sided ultrasound-guided ESP block (20 ml 0.2% ropivacaine and 4 mg dexamethasone) at the level of L3, since he only complained of pain on that side.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">Data were collected prospectively at 3 time points: (1) <span class="elsevierStyleItalic">before the intervention</span>: demographics, body mass index (BMI), clinical history, current pain therapy, VAS pain score, Brief Pain Inventory – Short Form (BPI)<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> and Neuropathic Pain Questionnaire (DN4)<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a>; (2) <span class="elsevierStyleItalic">30 min after the intervention</span>: VAS pain score and level of satisfaction with the technique (from 0: “totally dissatisfied" to 10: “totally satisfied"); (3) 24 h, 7 days and 1 month after the intervention (telephone call): complications and pain score (VAS).</p><p id="par0040" class="elsevierStylePara elsevierViewall">Data were collected and examined using Microsoft Excel® 15.41.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0045" class="elsevierStylePara elsevierViewall">We performed ultrasound-guided ESP block for CLP in 10 selected patients who were numbered from “1” to “10”.</p><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Patient characteristics and type of surgery</span><p id="par0050" class="elsevierStylePara elsevierViewall">All but 1 of our patients were women (n = 9, 90%). Age ranged from 52 to 88 years (mean = 70.3; standard deviation = 11.63). Body mass index was 27.211 kg/m<span class="elsevierStyleSup">2</span> (standard deviation = 4.71).</p><p id="par0055" class="elsevierStylePara elsevierViewall">Most of the patients had more than 1 chronic disease and were taking several different medications; 9 were taking more than 1 type of analgesic drug (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">All patients had chronic primary musculoskeletal low back pain, diagnosed according to the International Association for the Study of Pain classification of chronic pain for the International Classification of Diseases (ICD 11).</p><p id="par0065" class="elsevierStylePara elsevierViewall">The main symptom was lower back pain, although 9 patients (90%) also reported pain in other locations (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>, Q2). During physical examination, all patients presented paraspinal muscle tenderness and pain in the erector spinae muscle when pressure was applied at the lumbar level.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">Based on responses to the BPI questionnaire, 90% (n = 9) of patients had experienced severe pain (VAS ≥ 7) in the preceding 24 h, 40% (n = 4) reported VAS ≥ 4 as their minimum level of pain, 40% lived with severe pain, and the remaining 60% lived with moderate pain (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>, Q3, Q4, Q5). Patients reported an average of only 34% relief with pain medication. In terms of their daily activities, the general activity item received the highest score (mean 6.5 — <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>, Q9A). Relations with other people and sleep were the items least affected by pain (mean 3.3); (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>, Q9E and F).</p><p id="par0075" class="elsevierStylePara elsevierViewall">Although none of the patients manifested signs of radiculopathy, all reported 1 or more neuropathic descriptors, and 50% (n = 5) scored ≥ 4/10 points on the Neuropathic Pain Questionnaire (DN4) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">Responses to the Brief Pain Inventory –Short form and the Neuropathic Pain Questionnaire (DN4) are shown in <a class="elsevierStyleCrossRefs" href="#tbl0010">Tables 2 and 3</a>.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Post-procedural evaluation</span><p id="par0085" class="elsevierStylePara elsevierViewall">None of the patients presented complications during administration of the ultrasound-guided ESP block or over the course of the study. Immediate post-procedural outcomes are shown in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">Patients completed a telephone survey a 24 h, 72 h, 7 days and 1 month of the blockade. The results are shown in <a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>. Patient 2 stopped answering calls 1 week after the procedure.</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0095" class="elsevierStylePara elsevierViewall">Ultrasound-guided ESP block is considered a valid component of a multimodal analgesic protocol for acute and chronic thoracic, lumbar and abdominal pain<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,10,11</span></a>.</p><p id="par0100" class="elsevierStylePara elsevierViewall">This is a simple, safe technique based on easily identifiable ultrasound landmarks and clear visualization of LA spread<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a>. The tip of the needle is located far from the spine, so there is little risk of complications affecting this or other vital structures<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,12</span></a>. Potential risks such as pneumothorax, haematoma, and tissue damage have been described by Ahiskalioglu et al.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a>. Similarly, Tulgar et al.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> reported 4 cases of complications in their retrospective review of 182 patients undergoing ultrasound-guided ESP block, all of which were due to the spread of LA to the lumbar plexus or to systemic LA toxicity, which can occur when performing other types of regional anaesthetic techniques.</p><p id="par0105" class="elsevierStylePara elsevierViewall">A few recent case reports have shown the effectiveness of ultrasound-guided ESP block in chronic back pain, such as low back pain after back surgery<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a>, or myofascial pain syndrome<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a>. On the basis of these reports, we decided to perform ESP in our outpatient population, given their lack of response to conservative therapy and the pressing need to finding an alternative therapy to improve their quality of life.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Despite receiving daily doses of strong opioids, 9 of our patients had not achieved satisfactory control. Prior to ultrasound-guided ESP block, their mean pain score had been 6.1, with a mean score of 7 on the day they came to the Pain Clinic for their treatment. They also reported that long-term therapy had only slightly improved their pain (mean 34% pain relief). Their pain also interfered considerably with their daily activities, particularly their general activities. Relations with other people and sleep were the least affected items.</p><p id="par0115" class="elsevierStylePara elsevierViewall">In most patients we performed bilateral ultrasound-guided ESP block administering 20 ml LA each side. According to Takahashi et al.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> and Tulgar et al.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a>, the ideal volume of LA has yet to be established, but 30 to 40 ml were administered in most patients, with good outcomes.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Patients were very satisfied with the technique (mean score of 8.75 in the satisfaction questionnaire) and achieved immediate and extensive pain relief (mean VAS score of 2.3). However, patient number 2 reported a post-procedural VAS of 7. Although this patient was very satisfied with the procedure itself, it did not relieve his pain. There were no complications or side effects in this specific case.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Good outcomes were maintained at 24 and 72 h after treatment, with mean VAS scores of 3.2 and 3.1, respectively. Pain scores at 7 days and 1 month after ESP, with a mean VAS of 3.8 and 6.2, respectively, were obtained from only 9 patients (patient number 2 dropped out of the study). Overall, pain relief lasted 20.8 days (with a standard deviation of 8.48); in 1 case it lasted for just 3 days, and in the remaining cases it lasted ≥15 days. As mentioned above, no significant complications were observed.</p><p id="par0130" class="elsevierStylePara elsevierViewall">In a report on chronic low back after back surgery, Takahashi et al.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a>, reported pain relief lasting 10 h after the first nerve block. The procedure was repeated 3 times a month thereafter, and achieved >60% pain relief. In a patient with chronic chest pain, Fusco et al.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> achieved pain relief lasting10 days with ultrasound-guided ESP block, and Piraccini et al.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> achieved pain relief lasting 7 days after ESP block in the sacrum, which allowed them to administer additional treatment (injection of transforaminal steroid).</p><p id="par0135" class="elsevierStylePara elsevierViewall">In a case of chronic shoulder pain, Forero et al.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> achieved pain relief lasting up to 12 weeks. Interestingly, patient number 6 in our series reported relief of shoulder pain lasting more than 30 days after the procedure. Long lasting analgesia was also reported after placement of a catheter in the interfascial plane for continuous ESP block to treat chronic pain<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,17</span></a>.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Based on our study and all the case reports mentioned, we believe that ultrasound-guided ESP block has advantages for chronic low back pain: (1) it is easy to perform and has few complications; (2) the patient can be immediately discharged with no motor blockade; (3) the technique is 100% effective in the short and medium term (mean duration 20.8 days); (4) there is no need to step up systemic drugs, such as strong opioids; (5) it produces a good "therapeutic window" during which patients with tolerable levels of pain (interruption of the pain cycle) can undergo physical rehabilitation and exercise.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Based on each individual response to the single shot technique and the duration of pain relief, repeated injections or continuous catheter infusion could be considered to achieve more long-lasting pain control.</p><p id="par0150" class="elsevierStylePara elsevierViewall">An important limitation of our study is the presence of concomitant osteoarticular lumbar disease in our case series, particularly among the elderly patients. This made it difficult to confirm that their pain was purely myofascial.</p><p id="par0155" class="elsevierStylePara elsevierViewall">The main limitation of our study is the lack of randomization — the patients were selected by the authors, and the sample size was small. As this is not a case-control study, the scientific evidence presented has limited value. Prospective data collection: before the intervention –demographical data, past medical history, current pain therapies, VAS pain level, Brief Pain Inventory– Short Form and Neuropathic Pain Questionnaire; 30 min after – VAS and satisfaction level; 24 and 72 h, 7 days and 1 month after – complications and pain level.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0105" class="elsevierStyleSimplePara elsevierViewall">Majority of females (90%), mean age of 70.3 years-old. All had primary musculoskeletal CLBP. 90% experienced severe pain (VAS >= 7) in the last 24 h. Half presented neuropathic characteristics. Patients were very satisfied with the technique (mean: 8.75) with immediate pain relief (VAS mean: 2.3). 24 and 72 h, 7 days and 1 month after the treatment VAS means were 3.2, 3.1, 3.8 and 6.2.
