Once anaesthesia had been induced, an introducer was placed in the right subclavian vein. After the Swan-Ganz catheter had been placed, we experienced difficulty in withdrawing both the catheter and the introducer. An x-ray examination showed a knot at the distal end of the catheter (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>), making it necessary to remove the device surgically. The knot in the catheter involved the introducer, making any withdrawal manoeuvre impossible. Swan-Ganz catheter placement is not extent from complications, such as the formation of intravascular or intracavitary knots.
