According to the Spanish National Health Survey 2017<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> and the Catalonia Health Survey 2017,<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> 15% of over 65 s report suffering severe or extreme pain (9% of men and 21% of women). Primary Care (PC) is the gateway to the health system for the vast majority of patients, and pain is the primary reason for consultation in this setting, with more than 30% of consultations related to chronic pain.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> Individuals with generalized chronic pain have worse quality of life, poorer mental health, and higher cardiovascular risk, and place a greater economic burden on health services.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Pain medications are the most widely consumed drugs (25%).<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> Strong opioids (SO) are used for CNMP (morphine, fentanyl, hydromorphone, oxycodone, tapentadol and buprenorphine). The main indication for these drugs is cancer pain<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a>; their efficacy in CNMP has not been firmly demonstrated, and they are associated with a high risk of adverse events.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7–9</span></a> Opioid prescriptions are becoming commonplace in PC<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,11</span></a>: data on SOs in Catalonia show a sustained increase in the number of users (from 2.69 DDDs [defined daily dose per 1000 inhabitants per day] in 2012 to 4.01 DDDs in 2016, an increase of 67% in 4 years<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a>). Total consumption in Spain has increased from 2.85 DDD in 2010 to 8.09 DDDs in 2018.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> The increase in the use of ultra-fast-acting opioids such as fentanyl citrate (65% between 2011 and 2015<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>) is particularly alarming because of their high risk of addiction and adverse effects.</p><p id="par0015" class="elsevierStylePara elsevierViewall">In the US and Canada, opioid consumption has reached epidemic proportions in recent years.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13,14</span></a> Even taking into account the differences between the US and Spain,<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> this trend is a wake-up call that should compel us to take preventive measures and change our approach to CNMP and de-medicalise pain by incorporating non-pharmacological measures in pain management strategies.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Studies conducted in other countries have shown SO prescription patterns in primary care physicians (PCP) and their adherence to clinical guidelines,<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16–20</span></a> and surveys performed in these countries have shown the attitudes of PCPs towards SOs.<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">21–24</span></a> However, although we have data on the number of SO prescriptions in Spain, no studies have yet explored the attitude of PCPs towards these drugs.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Although it is reasonable to assume that interaction between PC and pain units (PU) would improve SO prescription practices and reduce risks,<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> the extent of this cooperation has not hitherto been analysed in Spain.</p><p id="par0030" class="elsevierStylePara elsevierViewall">In light of the foregoing, we designed a questionnaire to explore prescription patterns for SOs among PCPs, the challenges they face, and their relationship with referral PUs. Identifying the problems experienced by primary care physician in prescribing SOs will contribute to future strategies designed to improve this situation.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Design</span><p id="par0035" class="elsevierStylePara elsevierViewall">In this descriptive study, a self-administered questionnaire was sent by email to all PCPs who are members of the Catalan Association of Family and Community Medicine (CAMFiC).</p><p id="par0040" class="elsevierStylePara elsevierViewall">The questionnaire was based on surveys published in other countries<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">21–24</span></a> and the recommendations of clinical guidelines for the safe prescription of SO,<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,7,26–28</span></a> and was designed to answer the study objectives. The study was authorised by the Parc de Salut Mar Ethics Committee and registered under number 2018/7800/I.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The questionnaire was complied on Google docs® (Google, California, USA), and contained multiple choice questions, 5-point Likert-scale questions, and open-ended questions arranged in 4 sections: demographic data, prescribing practices, clinical practice, and relationship with referral PUs.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Study population</span><p id="par0050" class="elsevierStylePara elsevierViewall">Member of CAMFiC who are PCPs 3602 members and have an active email address registered on the Association’s database were included in the study. Twenty-nine CAMFiC members without an active email address were excluded. A contact email was required on the form as a security measure to avoid duplications.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Variables collected</span><p id="par0055" class="elsevierStylePara elsevierViewall">Demographic data (age, sex, health care district), years working in PC, number of patients assigned by the health system, and number of patients seen for CNMP and treated with SOs at the time of answering the survey.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Types of SOs prescribed and indications for use (multiple choice). In the open-ended question "What difficulties do you encounter at the start of SO therapy or during maintenance?" answers were grouped into 7 topics: SO management, reluctance, training, third part pressure to prescribe, adherence, no difficulty, and others. These groups were created according to the responses received in the completed questionnaires.</p><p id="par0065" class="elsevierStylePara elsevierViewall">SO prescription patterns were also recorded: pharmaceutical form and frequency of adverse effects (multiple-choice questions), rescue medication, dose titration, concomitant use of benzodiazepines and use of ultrafast-acting opioids (yes/no dichotomous responses), patients given ultrafast-acting opioids and indications for use (multiple choice).</p><p id="par0070" class="elsevierStylePara elsevierViewall">Four 5-point Likert questions were included: “Do you establish pain relief and functionality targets when starting SOs, and do you periodically evaluate effectiveness and side effects with the patient" (1 "never" - 5 "always"); «Do you feel comfortable using SOs?» (1 "extremely uncomfortable" - 5 "very comfortable"); "Do you think you have enough training to use them?" (1 "too little training" - 5 "enough training"), and "Relationship with the reference pain unit" (1 "very unsatisfactory" - 5 "very satisfactory").</p><p id="par0075" class="elsevierStylePara elsevierViewall">In the open-ended question “What is needed to improve the relationship with your PU?” the answers were grouped into 8 thematic blocks: communication, accessibility, consensus on the prescription, continuum of care, training, information on their portfolio of services, nothing, and others. These groups were created according to the responses received in the completed questionnaires.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Statistical analysis</span><p id="par0080" class="elsevierStylePara elsevierViewall">Categorical variables are expressed as absolute frequency and percentages (%). Continuous variables are expressed as means (standard deviation). Statistical analysis was performed using the R version 3.2.3 for Windows.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Results</span><p id="par0085" class="elsevierStylePara elsevierViewall">The email was first sent to the 3602 PCPs members of CAMFIC on March 15, 2018. A total of 400 responses were received within 3 weeks, and after a reminder had been sent, 551 responses were received within 1 month of the first email.</p><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0140">Sociodemographic data (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>)</span><p id="par0090" class="elsevierStylePara elsevierViewall">The questionnaire was completed by 551 PCPs (15.3% response rate), with an average age of 46.0 years (SD 9), of which 443 (76.6%) were women. The respondent profile corresponded to a female PCP with 10–20 years of experience in P, working in the metropolitan Barcelona health district, with 1500-2000 assigned patients.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0145">Use of strong opioids (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>)</span><p id="par0095" class="elsevierStylePara elsevierViewall">The most frequently prescribed SO was fentanyl (499 respondents, 90.6%). Other SOs, such as morphine, buprenorphine, oxycodone and tapentadol, were routinely used by 24.1% (133) to 32.7% (180) of PCPs. Less than half (39%, 215 respondents) did not consult the PU after reaching high doses of transdermal fentanyl (even 100 µg/h or more).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">Almost half (48.6%, 268 respondents) reported having between 1 and 4 patients on ultrafast-acting (transmucosal) fentanyl. Of these, 6.3% (17 PCPs) prescribed it only for CNMP. Less than half (41.8%, 112 respondents) did not always associate transmucosal fentanyl with chronic opioid therapy.</p><p id="par0105" class="elsevierStylePara elsevierViewall">The mean score for the questions “Do you feel comfortable using SOs?” and “Do you think you have enough training to use them?” was 3.2 ± 0.9 in both cases.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0150">Clinical practice (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>)</span><p id="par0110" class="elsevierStylePara elsevierViewall">Nearly all (87.1%, 480 respondents) prescribed SOs for musculoskeletal pain, and 18.1% (100) for fibromyalgia; 64.1% (353) always titrated the dose, with 35.6% (196) using quick-release formulas. Rescue doses were occasionally prescribed by 58.1% (320) of PCPs. A total of 75.3% (415) attempted to down-dose benzodiazepines when prescribing SOs, while 52.3% (288) occasionally prescribed laxatives in SO users, and 31.2% (177) always did so.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0115" class="elsevierStylePara elsevierViewall">The mean score for the question “Do you establish pain relief and functionality targets when starting SOs, and do you periodically evaluate effectiveness and side effects with the patient" (1 "never" - 5 "always") was 3.9 ± 0.9.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Side effects and their frequency are shown in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> ("never" = 1, and "almost always” = 5).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0125" class="elsevierStylePara elsevierViewall">The mean score for use of the visual analogue scale (VAS) to assess pain was 3.9 ± 1.1 (1 = "never" - 5 = "always").</p><p id="par0130" class="elsevierStylePara elsevierViewall">The responses to question about difficulties in SO management are shown in <a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>. Most (71%, 341 respondents) reported problems in the daily management of SOs, including adverse effects (17.2%, 83), opioid tolerance (12%, 58) or pain control (9.1%, 44), whereas 9.8% (47) reported no or few difficulties in SO management.</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0155">Relationship with the pain unit</span><p id="par0135" class="elsevierStylePara elsevierViewall">The mean score for the relationship with the reference PU was 2 ± 1. The suggestions put forward to improve this relationship are shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>. Half (52.2%, 271 respondents) indicated <span class="elsevierStyleItalic">lack of communication</span> as the main problem, and the improvements put forward included virtual consultations, meetings and visits from PU staff to PC centres, PCP rotations in PUs, or shared clinical training. Some (27.1%, 141 respondents) mentioned difficulties in <span class="elsevierStyleItalic">referring patients to</span> PUs, including excessive waiting times for both the first visit and for scheduling procedures, while 6.5% (34) were unable to refer paints to a PU.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0160">Discussion</span><p id="par0140" class="elsevierStylePara elsevierViewall">This study describes SO prescription patterns in CNMP in our setting based on responses to an email survey. PCPs adhere to clinical guidelines in some respects, such as down-dosing benzodiazepines, the use of VAS, or dose titration, but not in others, such as consulting the PU when using high doses of SOs, prescribing laxatives, or avoiding SOs in patients with fibromyalgia. A large number of patients received ultrafast-acting opioids for unapproved indications, and without chronic opioid therapy. PCPs demand more training, are reluctant to prescribe SOs, and see the need to improve their relationship with PUs.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Respondents reported seeing a lower percentage of patients with CNMP than reported in other studies, where around 30% of patients are seen for CNMP.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> In line with other studies, fentanyl is the most widely used SO,<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> but the high percentage of PCPs using morphine and oxycodone appears to contradict official reports,<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,12</span></a> which show much lower prescription percentages (9% for oxycodone and less than 5% for morphine in Catalonia, with similar percentages nationwide).</p><p id="par0150" class="elsevierStylePara elsevierViewall">One third (39%) of respondents do not consult the PU when using high doses of transdermal fentanyl. This is cause for concern, since guidelines<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,8–10,27,28</span></a> advise against exceeding 100 OME (oral morphine equivalent, 50 µg fentanyl), and recommend consulting the reference PU if high doses are needed. However, not all PCPs have access to a pain unit.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Two hundred and sixty eight (48.6%) respondents prescribe ultrafast-acting fentanyl in some patients. This high percentage shows the importance of raising awareness among PCPs of the indications and risks of this drug<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,9</span></a>: 44 (17.4%) respondents always or nearly always prescribe it for CNMP, even though this indication in unapproved and carries a risk of addiction<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a>; and 38 (14.1%) do not prescribe ultrafast-acting fentanyl for breakthrough pain in chronic SO therapy, when this is essential to reduce the risk of addiction.</p><p id="par0160" class="elsevierStylePara elsevierViewall">A total of 480 (87%) PCPs prescribe SOs for musculoskeletal pain, and 237 (43%) for neuropathic pain. Although there is little evidence of the effectiveness of SOs in CNMP, they are contraindicated in some types of pain, such as fibromyalgia (for which 100 [18%] PCPs prescribe SOs), visceral pain or headaches (given by 133 [24%] for “other pain”).<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a></p><p id="par0165" class="elsevierStylePara elsevierViewall">A total of 353 (64%) respondents always titrate the SO dose, although only 196 (35.6%) do so with quick-release formulas, and contrary to recommendations,<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,28</span></a> only 100 (18%) routinely prescribe rescue doses. Four hundred and thirty four (78.7%) respondents down-dose benzodiazepines when starting SOs. Although SOs and benzodiazepines should not be given in combination,<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,8,10,26</span></a><span class="elsevierStyleSup">,</span><a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> the fact that most respondents attempt to down-dose benzodiazepines is a positive sign.</p><p id="par0170" class="elsevierStylePara elsevierViewall">A total of 288 (52%) respondents occasionally prescribe laxatives, and only 172 (31%) always do so, even though consensus documents<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,27</span></a> recommend prescribing them systematically when starting SOs, and constipation is the adverse effect most widely reported by PCPs. The second most common adverse effects after constipation are nausea and vomiting and drowsiness. Neurotoxicity, itching and addiction are occasionally detected, and respiratory depression rarely appears.</p><p id="par0175" class="elsevierStylePara elsevierViewall">Contrary to our expectations, many PCPs report using additional strategies, such as VAS and doctor-patient agreements (establishing pain relief and functionality targets before starting treatment, with joint monitoring during therapy), considered safe practices in the prescription of SOs.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,26,28</span></a> This is a promising development, given the percentages reported in other studies (Johnson et al.,<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> for example, reported 48% of doctors using VAS). The lack of confidence and training in the use of SOs is consistent with other studies (85% of respondents in the study by Blake et al., 89% in the study by Johnson et al.),<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">21,29</span></a> and confirms the need for more training.</p><p id="par0180" class="elsevierStylePara elsevierViewall">When asked to describe the difficulties encountered at the start of SO therapy or during maintenance, most respondents mention reluctance on the part of both doctors and patients, an attitude also observed in other studies (67.8% in the study by Blake et al.).<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">21,30</span></a> Even though many PCPs are concerned about addiction (76% in Blake et al.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a>), their fears do not appear to be grounded in reality, given the low incidence of addiction in clinical practice.</p><p id="par0185" class="elsevierStylePara elsevierViewall">The general dissatisfaction shown by PCPs in respect of their relationship with PUs highlights to need to strengthen this collaboration in order to improve management of CNMP and SO at the primary care level.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,7,26</span></a> Most respondents insist on the need to encourage or increase interaction with PUs, and improve patient access to these units by shortening waiting lists. Respondents also highlight the need for shared prescribing protocols for drugs with approved indications.</p><p id="par0190" class="elsevierStylePara elsevierViewall">This study has some limitations: although the percentage of participation was high compared to similar studies,<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">21–24</span></a> reflecting the current interest in the subject, the sample size may not be representative of all PCPs, and there is a risk of response bias. Although the response rate is not high, the number of responses received validates our results, since it is higher than most similar studies reviewed, and more than 500 responses is a representative sample of Catalan PCPs, with an acceptable margin of error.</p><p id="par0195" class="elsevierStylePara elsevierViewall">Selection bias cannot be ruled out, since members of scientific societies are more motivated than non-members due to their interest in continuing professional development. Finally, the study is prone to the typical limitations of Likert-scale questions, which force respondent to choose 1 of 5 options.</p><p id="par0200" class="elsevierStylePara elsevierViewall">Further research should focus on the use of a combination of weak and strong opioids (a practice that is not recommended), approaches to withdrawing patients from SOs, adverse effects according to the type of SO, or the combination of benzodiazepines, alcohol consumption and SOs.</p><p id="par0205" class="elsevierStylePara elsevierViewall">In a subsequent phase, we will obtain real SO prescription data from Catalan PCPs that, together with the data obtained in this survey, will help us prioritize areas of improvement. Publishing these results and reporting on the study in conferences will raise awareness among Catalan PCPs and guide the creation of training courses designed to correct specific prescribing patterns. Safe SO prescribing patterns should be prioritised in order to prevent the current opioid epidemic in the US and Canada, caused by pressure from the pharmaceutical industry, the medicalization of society, recreational use of SOs, etc. to spread to Spain.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13,14</span></a> Although the situation in Spain and the US differ in many respects, we must not lose track of the steady increase in SO use, and action must be taken to remedy the key deficiencies in clinical practice guidelines for SO in primary care in Catalonia revealed by this survey. In this regard, we must applaud initiatives such the position paper published by the Medical Association of Catalonia<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> which, in collaboration with different scientific associations, puts forward recommendations for the rational use of SOs.</p></span><span id="sec0065" The assessment of the relations with the Pain Management Units was 2 ± 1 (on a 1–5 scale), with communication (271, 52.2%) and accessibility (141, 27.1%) as the areas most in need of improvement.</p></span> <span id="abst0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Conclusions</span><p id="spar0070" class="elsevierStyleSimplePara elsevierViewall">The opioid prescribing patterns mostly follow clinical guidelines (e.g. reduction of benzodiazepine use or dose titration). However, there are some areas for improvement, like little use of laxatives or use of ultra-rapid opioid for unapproved indications and without a basic opioid treatment. Los efectos adversos más habituales son estreñimiento y náuseas. Las principales dificultades observadas en la prescripción son su manejo (341, 71%) y la resistencia de pacientes y profesionales (87, 18.1%). La valoración de la interrelación con las Unidades de Dolor fue 2 ± 1 (escala 1 al 5), siendo problemas en la comunicación (271, 52.2%) y en la accesibilidad (141, 27.1%) los principales puntos de mejora.</p></span> <span id="abst0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Conclusiones</span><p id="spar0110" class="elsevierStyleSimplePara elsevierViewall">Los patrones de prescripción se adecuan mayoritariamente a las guías clínicas en algunos aspectos (disminución de benzodiacepinas o titulación de dosis). Sin embargo existen áreas de mejora como son el poco uso de laxantes o el uso de opioides ultrarrápidos para indicaciones no autorizadas y sin tratamiento opioide de base.
