The preoperative chest X-ray (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>) showed some alterations that were not initially considered significant in the pre-anaesthesia evaluation.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">However, when reviewing previous radiographic studies to determine the size and possible implications of the hiatal hernia, we found an x-ray that had been performed 6 months earlier to assess the spine that showed a large hiatal hernia seen as an air-fluid level that overlapped the cardiac silhouette (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). On the basis of these findings, we changed our anaesthesia approach in order to avoid pulmonary aspiration during intubation manoeuvres.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The comparison of both radiographs makes us reflect on the importance of the technique to reach a specific diagnosis.
