The preoperative chest X-ray (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>) showed some alterations that were not initially considered significant in the pre-anaesthesia evaluation.<elsevierMultimedia ident="fig0005"></elsevierMultimedia>However, when reviewing previous radiographic studies to determine the size and possible implications of the hiatal hernia, we found an x-ray that had been performed 6 months earlier to assess the spine that showed a large hiatal hernia seen as an air-fluid level that overlapped the cardiac silhouette (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). On the basis of these findings, we changed our anaesthesia approach in order to avoid pulmonary aspiration during intubation manoeuvres.The comparison of both radiographs makes us reflect on the importance of the technique to reach a specific diagnosis. Revista Española de Anestesiología y Reanimación (REDAR) -the Spanish Journal of Anesthesiology and Resuscitation- is the scientific body of the Spanish Society of Anesthesiology, Resuscitation and Pain Therapy. This is a monthly journal (10 issues) that publishes scientific articles of all areas within its specialty: clinical anesthesia, resuscitation-intensive medicine and critical care, diagnosis and treatment of acute and chronic pain, urgent care and emergencies and studies on basic science and related fields. The journal accepts work in both Spanish and English.
