CE: conventional extubation; EuFT: ultra fast-track extubation.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Fast track and ultra fast track extubation (EuFT) protocols are designed to optimise care processes and facilitate rapid recovery after cardiac surgery, including the pre-, intra- and postoperative periods.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The increase in the number of surgeries performed, together with the need to maximise efficiency in clinical practice, has led to the implementation of safe, fast track strategies in cardiac care, such as anaesthesia with a combination of low-dose, short acting hypnotics and low-dose opioids<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">3</span></a> that permits early postoperative extubation.<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">4–6</span></a> The latter strategy (EuFT) is now considered to be one of the cornerstones of efficient practice, and different studies have shown that it can reduce hospital stays and time spent in the intensive care unit (ICU), and has a direct impact on costs and care quality.<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">6–8</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Despite all the scientific evidence, protocols vary greatly between hospitals, and fast track and EuFT criteria and step by step protocols have yet to be standardised on an international level. The aim of this study, therefore, has been to describe and compare the duration of ICU and hospital stay together with postoperative mortality rates in patients undergoing EuFT vs. conventional extubation (CE) after cardiac valve surgery, closure of cardiac defects, coronary bypass and mixed surgery in a cardiovascular reference centre in Colombia.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">This was a retrospective, longitudinal, analytical study in patients aged over 18 years undergoing valvular surgery, closure of cardiac defects, coronary bypass or combined surgeries using both conventional and minimally invasive techniques. All patients treated in a tertiary level hospital in Colombia from April 2009 to December 2015 were included in the study. Patients undergoing reoperation and those that did not survive surgery were excluded.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The study was evaluated by the Research Ethics Committee of the Universidad Pontificia Bolivariana, and was approved on 23 May 2016. The authors have safeguarded the confidentiality of all patient data.</p><p id="par0030" class="elsevierStylePara elsevierViewall">No fast-track measures were used in patients who received EuFT. The following standard hospital anaesthetic protocol was followed in all cases: anaesthetic induction with midazolam, fentanyl and vecuronium; intubation and invasive monitoring with arterial line and central venous catheter; anaesthesia maintained with fentanyl infusion and inhalational sevoflurane.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The decision to extubate was not guided by an established protocol, but instead was based on intraoperative haemodynamics and ventilation. The opioid and muscle relaxant strategy did not depend on whether the patient was extubated in the operating room or the ICU. For statistical purposes, patients who were extubated at the end of the surgery, that is, in the operating room before being transferred to the ICU, were assigned to the EuFT group. Patients extubated in the ICU were assigned to the CE control group.</p><p id="par0040" class="elsevierStylePara elsevierViewall">All the relevant variables were recorded in the database according to the duration of surgery. Preoperative variables included clinical and demographic data, such as age, sex, weight, height. Also included were heart disease-related comorbidities, such as hypertension, diabetes, chronic obstructive pulmonary disease, congestive heart failure, valvular heart disease, congenital heart disease, kidney failure, peripheral vascular disease, previous ejection fraction, coronary involvement, preoperative risk classification according to the American Society of Anesthesiology (ASA), functional status according to the New York Heart Association (NYHA), and airway evaluation. Data from the intraoperative period included dose of midazolam, fentanyl and vecuronium, infusion time, aortic clamping time, volume of intraoperative fluids and red blood cells, plasma, cryoprecipitate and platelets administered. The surgical technique was recorded as minimally invasive surgery or off-pump surgery, and Swan-Ganz right heart catheterization. Postoperative variables included length of hospital and ICU stay, cardiac (myocardial ischaemia), kidney (acute kidney injury), neurological (stroke) or respiratory (pulmonary vascular congestion shown on postoperative chest X-ray) complications, need for vasopressor or vasodilator drugs, and postoperative mortality.</p><p id="par0045" class="elsevierStylePara elsevierViewall">To classify outcomes, myocardial ischaemia was defined as postoperative ST segment depression and T wave inversion, and postoperative inotropic support was defined as the need for norepinephrine at a starting dose of 0.05<span class="elsevierStyleHsp" style=""></span>mcg/kg/min to maintain mean arterial pressures above 70<span class="elsevierStyleHsp" style=""></span>mmHg.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The data were processed and analysed using SPSS, version 20.0. Statistical significance was set at 5% (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05). The Kolmogorov–Smirnov and Shapiro–Wilk tests were used to evaluate the normality of the different continuous variables of both groups as a whole, and of the different subgroups, namely, EuFT and CE.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Clinical, sociodemographic and surgical variables were described as appropriate. Normally distributed continuous variables were described as mean plus standard deviation; all the remaining variables were described as median plus interquartile range. In the case of nominal variables, the percentage of individuals with and without the characteristic evaluated was calculated. Normally distributed continuous variables were compared between groups (EuFT and CE) using the Student's <span class="elsevierStyleItalic">t</span> test. Comparisons between non-normally distributed variables were performed using the Mann–Whitney <span class="elsevierStyleItalic">U</span> non-parametric test. The Chi-square test (<span class="elsevierStyleItalic">χ</span><span class="elsevierStyleSup">2</span>) or Fisher's exact test were used on categorical variables, and the prevalence ratios with their respective 95% confidence intervals (CI) were calculated. Finally, to estimate the effect size measure as an odds ratio (OR) calculated using logistic regression, and to identify the effect of the extubation strategy on each clinical outcome evaluated (cardiac [myocardial ischaemia], kidney [acute kidney injury], neurological [stroke] or respiratory [pulmonary vascular congestion shown on postoperative chest X-ray] complications, need for vasopressor or vasodilator drugs, and postoperative mortality), we performed a stepwise multivariate analysis that included all variables that were shown to be statistically significant in the bivariate analysis of extubation groups and clinical, demographic and intraoperative variables.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0060" class="elsevierStylePara elsevierViewall">As shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>, a total of 396 patients undergoing valvular surgery, closure of cardiac defects, coronary bypass or combined surgery were included in the study. Of these, 207 underwent EuFT and 189 CE. The sample selection protocol is shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">As shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>, the groups were not evenly matched, due to statistically significant differences in age, ejection fraction, NYHA classification and history of congenital heart disease, diabetes and hypertension. We found that patients undergoing EuFT were generally younger, with a better functional class, fewer comorbidities, and better ejection fraction. Only 17.4% of patients undergoing EuFT had an ejection fraction of less than 45%, while the ejection fraction of 29% of subject undergoing CE was even lower.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">With respect to intraoperative variables shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>, statistically significant differences were found between both groups in the percentage of transfusions: only 14.4% of EuFT patients were transfused vs. 28.9% of patients with CE. EuFT was performed in a higher percentage of patients undergoing minimally invasive surgery and heart defect surgery, while CE was performed in a high percentage of patients undergoing combined surgery.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Although no statistically significant differences were found between groups in terms of postoperative mortality, the CE group presented higher cardiovascular morbidity, namely myocardial ischaemia (95% CI: 0.37–0.86; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.01), and greater need for postoperative vasopressor support (95% CI: 0.18–0.49; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.01). As shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>, after adjusting multivariate statistics for the influence of other variables on clinical outcomes, we found that EuFT maintained its statistical independence and was associated with a lower incidence of cardiac complications, which were higher in the CE group (95% CI: 0.28–0.73; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001), and less need for postoperative vasopressor support (95% CI: 0.23–0.71; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">In order to estimate effect size, ORs were obtained by binary logistic regression for each of the following dependent variables: postoperative mortality, postoperative respiratory, neurological, kidney, and cardiovascular complications, need for postoperative vasopressor drugs, and need for postoperative vasodilator drugs. All clinical, demographic and surgery-related covariables found to be statistically significant in the bivariate comparison of groups, namely, age, ejection fraction, history of congenital heart disease, diabetes, hypertension, NYHA classification, type of surgery, minimally invasive surgery, and total number of transfusions, were included. <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the clinical outcomes according to extubation group, expressed both as gross OR values and OR values adjusted for the aforementioned post-multivariate analysis variables.</p><p id="par0085" class="elsevierStylePara elsevierViewall">As shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>, no statistically significant differences were found in respiratory, neurological or kidney complications. Length of hospital stay was similar in both groups: a total of 6 days (IQR 5–8 for EuFT and IQR 5–10 for CE, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.16), as was overall length of ICU stay (24<span class="elsevierStyleHsp" style=""></span>h), with no statistically significant differences between groups (RIQ 18–36 for EuFT and RIQ 24–48 for CE, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.09).</p><p id="par0090" class="elsevierStylePara elsevierViewall">Although patients extubated in the operating room (EuFT) presented a lower comorbidity burden and better pre-operative functional status than the CE group, EuFT remained an independent factor for a lower rate of postoperative myocardial ischaemia and less need for postoperative vasopressor drugs. In terms of the 2 latter outcomes, the multivariate analysis specifically showed that, in addition to the type of extubation, only minimally invasive surgery remained a statistically independent beneficial factor for postoperative myocardial ischaemia, with an adjusted OR of 0.56 (95% CI: 0.35–0.89). In the same analysis, in addition to the site of extubation, only the type of surgery remained a statistically independent beneficial factor for the need for postoperative vasopressor drugs. Patients undergoing combined surgery presented greater need for vasopressor drugs, with an adjusted OR, taking as a reference point closure of defects, which was the procedure with the least vasopressor requirement (OR 6.6, 95% CI: 1.46–29) and history of diabetes (adjusted OR 3.6, 95% CI: 1.6–8).</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0095" class="elsevierStylePara elsevierViewall">We believe that EuFT after cardiovascular surgery in patients with no major comorbidities (good ventricular function, ejection fraction >45%, aged under 75 years, and no lung disease<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">5</span></a>) is a valuable anaesthesiology strategy for improving patient safety. Although it had no impact on postoperative mortality, it was associated with fewer cardiac complications, such as myocardial ischaemia, and a less need for postoperative norepinephrine.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Studies in fast-track surgery and EuFT have shown that early extubation alone has only a marginal benefit on costs, and is not <span class="elsevierStyleItalic">per se</span> a valuable strategy. The best savings are achieved by adopting different recovery strategies, such as complete fast-track and EuFT protocols that include pre-, intra- and postoperative measures; these will have the most important impact in the future. This means that switching to fast-track anaesthesia must be accompanied by changes in patient recovery models and strategies. This study has shown that EuFT does not reduce the length of hospital stay, ICU stay, or mortality, and therefore has no significant impact on care costs.</p><p id="par0105" class="elsevierStylePara elsevierViewall">The clinical evidence in this regard has shown that fast-track protocols<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">10</span></a> consistently reduce ICU<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">6,8–10</span></a> and hospital stays<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">6,8</span></a>; however, none of these studies has managed to find statistically significant differences between patients who received EuFT and CE<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">11</span></a> in terms of outcomes, including risk of postoperative mortality in the ICU<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">6–8</span></a> risk of 30-day mortality,<a class="elsevierStyleCrossRefs" href="#bib0095"><span class="elsevierStyleSup">5,9</span></a> risk of myocardial ischaemia,<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">6,8</span></a> and risk of reintubation at 24<span class="elsevierStyleHsp" style=""></span>h after surgery.<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">6,8</span></a> A study<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">12</span></a> in 100 patients showed a shorter hospital and ICU stay in the EuFT group, with a 53% reduction in intensive care costs, and 25% in surgical costs compared to the CE group. The meta-analysis published by Myles<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">7</span></a> in 1800 patients from 10 studies, found a reduction in ICU stay and a non-significant reduction in hospital stay and mortality. In a recent study, Richey<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">13</span></a> found no reduction in the length of hospital stay between the EuFT and CE groups (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.16), no reduction of ICU stay (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.09), and no difference in mortality (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.63). Furthermore, we believe that the timing of extubation is not just a measure of care quality, but should also be used to evaluate outcomes.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">14</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">In terms of other parameters, in contrast to studies that found no statistically significant differences in the risk of myocardial ischaemia,<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">6,8</span></a> we found a higher risk in the CE group (95% CI: 0.37–0.86; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.01) and observed minimally invasive surgery and EuFT to be protective factors for postoperative myocardial ischaemia. However, we found a history of diabetes, CE, and combined surgery to be risk factors for a greater need for vasopressive drugs.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Fast-track extubation protocols include extubation in the first 6 postoperative hours,<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">13,14</span></a> which includes extubation in the operating room. In some hospitals, the ICU is located at a distance from the operating room, so all the risks involved in the transfer must also be taken into consideration. In these circumstances, therefore, it is preferable to transfer the patient to the ICU before extubation to avoid airway complications during transfer, such as respiratory depression, residual relaxation, bronchoaspiration, or laryngospasm. The decision to extubate must be taken on an individual basis, taking into account the extent to which hospital infrastructure can optimise and guarantee patient safety.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Finally, despite the limitations of this study and the fact that the full fast-track protocol was not used, fast-track strategies in cardiac surgery reduce the rate of postoperative complications, particularly cardiovascular morbidities, and the need for post-operative vasopressor support. With their use, the length of hospital and intensive care unit (ICU) stays are reduced, with a direct impact on costs and the quality of the health service.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Objective</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">To compare the length of stay in the ICU, length of hospital stay, and post-operative mortality in ultra-fast-track extubated (uFTE) patients and those with conventional extubation (CE) after cardiac surgery.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Methods</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Longitudinal, analytical, retrospective study was conducted, with the period between the time of surgery and discharge being included as the study period.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Results</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">A total of 396 patients older than 18 years who required cardiac surgery were included, of whom 207 patients had (uFTE) and 189 had CE. Although the groups were not comparable due to the statistical differences found, when performing the multivariate adjustment, uFTE maintained its statistical independence and was associated with lower cardiovascular morbidity, such as myocardial ischaemia (95% CI: 0.37–0.86; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.01) and lower post-surgical vasopressor requirement (95% CI: 0.18–0.49; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.01).
