We need to evaluate the effectiveness of medical care in these patients in terms of morbidity, mortality and costs. This will allow us to make more rational decisions, set priorities, and establish protocols for each particular group of patients. For this reason, research has been conducted into developing prognostic scales,<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">1</span></a> which are tools designed to predict patient evolution, mainly mortality, based on clinical features and the associated surgical setting. They are not suitable for decisions at an individual level, but can be used to evaluate groups of patients with similar characteristics. As such, they are an excellent clinical care quality control tool.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Prognostic scales were first used in the general setting of severely ill patients. Perhaps the most important in this context are the American Acute Physiologic and Chronic Health Evaluation (APACHE) and the European Simplified Acute Physiologic (SAPS) scales. The most important cardiac surgery prognostic scale is the Euroscore I and the latest version, Euroscore II.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">2–4</span></a> These scales are only valid for certain time, and the latest cardiac surgery prognostic scales must improve the predictive power of their predecessors, and provide a more reliable risk estimate than general purpose prognostic scales. The objective of this study is to validate the Euroscale II in our patients, and thus evaluate whether it is an improvement over its predecessor and the general SAPS III scale. Our secondary objective has been to determine the group of surgical patients in which the scale was most effective.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0015" class="elsevierStylePara elsevierViewall">This is a retrospective cohort study conducted over a 3-year period (2011–2013), in which all patients aged over 18 years admitted to the Critical Cardiac Surgery Unit (CCCU) of the Virgen de la Salud hospital in Toledo were included. We excluded patients that could not be evaluated using any of the study disease severity scoring systems, patients readmitted during the study period (only data from the first admission are included), and patients referred from other units with more than 24<span class="elsevierStyleHsp" style=""></span>h of postoperative evolution.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Medical records (electronic or paper-based) from admission to hospital discharge were reviewed. Data collected included the origin of the patient, variables needed to calculate prognostic scales and evolutionary data (stays, complications and hospital discharge or exitus). Mortality according to the Euroscore I and II and SAPS III was calculated. To evaluate the different types of surgery, patients were classified as: coronary surgery, valvular surgery, combined surgery (coronary artery by-pass graft [CABG] and valve surgery) and other surgeries.</p><p id="par0025" class="elsevierStylePara elsevierViewall">We submitted the study to the research ethics committee of our hospital for approval. Data confidentiality was maintained in accordance with current legislation.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0030" class="elsevierStylePara elsevierViewall">Qualitative variables are expressed as number (percentage), and quantitative data as median (interquartile range). The SAPS III and Euroscore I and II scales were validated by determining the level of discrimination and calibration of each scoring system and their power of prediction compared with observed mortality.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Discrimination measures the ability of a system to correctly classify patients in terms of prognosis, distinguishing between survivors and non-survivors. For this purpose, we constructed receiver operating characteristic curves, which measure the model's discrimination in terms of its sensitivity and specificity for all possible values, and shows the diagnostic accuracy of the system. The area under the curve (AUC) shows the likelihood of having correctly classified a patient in terms of outcome, with 1 being the perfect prediction.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Calibration is a measure of the extent to which the probability of in-hospital mortality estimated by the model corresponds to the observed mortality rate. It is tested using the Hosmer–Lemeshow goodness of fit test. For this purpose, we grouped the observed and predicted values by deciles of measured probability, estimating the Chi square parameter with 8 degrees of freedom. Unlike other tests, non-significance indicates a better goodness of fit.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Statistical significance was set at <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05. Statistical analysis was performed using SPSS<span class="elsevierStyleSup">®</span> v. 20.0.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0050" class="elsevierStylePara elsevierViewall">Eight hundred and sixty-six patients were included in the study. Of these, 62.5% were men, with a median age of 69 years (60–76 years). In terms of admission, 70.7% were admitted for elective surgery, 25.6% for expedited surgery, 2.3% for emergency surgery, and 1.4% for life-saving surgery. By type of surgery, 27.7% underwent coronary surgery, 49.5% underwent valvular surgery, 13.7% underwent combined surgery (CABG plus valve), and 9% underwent other surgeries. Mean time on mechanical ventilation was 8<span class="elsevierStyleHsp" style=""></span>h (6–11), with a mean stay in the CCCU of 72<span class="elsevierStyleHsp" style=""></span>h (48–96). Cardiac complications (most commonly atrial fibrillation) were observed in 19.6% of patients, and 17.4% presented non-cardiac complications (most commonly kidney failure). A small percentage (2.8%) underwent reoperation. Mean post-CCCU hospital stay was 5 (4–7) days. Only 6.1% patients died during their hospital stay. Of these, 39% died in the operating theatre, 49% in the CCCU, and 12% on the hospital ward. By type of surgery, deaths occurred in 1.7% of coronary surgery patients, 4.9% of valvular surgery patients, 7.6% of CABG plus valve surgery patients, and 24.4% of patients undergoing other surgeries.</p><p id="par0055" class="elsevierStylePara elsevierViewall">An analysis of the value and predictive power of the prognostic scales studied allowed us to see the extent to which different prognostic scales are able to predict mortality in the same group of patients (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). We observed that predicted mortality in our sample was 2.1% on the Euroscore 1 scale, 4.99% on the Euroscore II scale, and 9.9% on the SAPS III.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">In the discrimination study, we studied the AUC, with the corresponding confidence interval and level of significance, of each scale for the whole sample and for each type of surgery (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). The Euroscore I and II scales showed good or excellent discrimination in all groups of patients, while the SAPS III showed poor discrimination in patients with combined CABG plus valve surgery (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">We used the Hosmer–Lemeshow goodness of fit test to evaluate calibration. The sample was divided into 10 groups (predictive deciles), in each of which predicted and observed survivors and non-survivors were evaluated. We observed adequate overall goodness of fit in the Euroscore I and SAPS III scales (not statistically significant), and poor goodness of fit in the Euroscore II scale (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.0001) (<a class="elsevierStyleCrossRefs" href="#tbl0015">Tables 3 and 4</a>). The Euroscore I was well calibrated in all patients, the Euroscore II in none, and the SAPS III in patients undergoing coronary and combined CABG plus valve surgery.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a> shows overall and surgery-specific predicted mortality of the 3 surgical groups.</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion</span><p id="par0075" class="elsevierStylePara elsevierViewall">Cardiac patients and cardiac surgery are highly idiosyncratic. Risk profiles in this population have changed considerably, and patients are now increasingly older, with greater comorbidity, and the number of reoperations and unscheduled interventions has also increased.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">5</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">Prognostic scales allow us to objectively evaluate the clinical status and disease severity of critically ill patients. They are an excellent measure of care quality in units treating complex patients, and provide the basis for comparing units located in different centres. These scales have various uses: they can assess quality of care, stratify patients for clinical studies, calculate patient-related costs, etc.,<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">6</span></a> and they also provide insight into the patient's prognosis in terms of predicted mortality (numerical percentage). The most commonly used scales in general critical patients are the different versions of the APACHE, SAPS and Mortality Prediction Model.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">7</span></a> One of their main limitations, however, is that they are unreliable when applied to populations not included in the studies on which these models are based. A comparison of older and new scales shows a definite improvement in performance in the latter. Unless severity scales are revised, updated and recalibrated in parallel with improvements in treatment and techniques, they run the risk of losing calibration and discrimination, and tend to overestimate mortality.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">8</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">In this study, we chose the SAPS III as a general scale because it was developed in a multicentre, Europe-wide study, and is based on data collected at the time of admission. One of the items included in SAPS III concerns patients that have undergone cardiac surgery, and this allowed us to verify the reliability of the scale to predict mortality in this group. The new version also includes the length of hospital stay before transfer to the ICU, thus avoiding lead time bias. It is considered one of the best prognostic systems available today, and it is free.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Cardiac surgery is a clear example of the change in attitude from a paternalistic doctor-patient relationship to a contractual relationship based on expected outcomes. This is why prognostic scales have been developed in this field to identify populations with a higher incidence of complications or greater likelihood of successful outcome following certain treatments. Several such scales have been created (Parsonnet, Society of Thoracic Surgeons, etc.), but the model most widely used in our setting is the Euroscore.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">2,3</span></a> The first version of this scale proved useful as a predictor of complications, longer ICU stay and medium- to long-term survival.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">9</span></a> Since then, several studies have highlighted its shortcomings, such as underestimating mortality in patients undergoing valve, coronary and CABG plus valve surgery, overestimating mortality in octogenarians, and poor specificity in thoracic aortic surgery. Although it has preserved its discrimination, it has gradually lost calibration over time and was therefore updated in the new Euroscore II.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">10</span></a> Since its publication, the Euroscore II has been externally validated by numerous authors.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Most published studies observe a discrete improvement in discrimination compared to the Euroscore I. Calibration results, however, are less favourable, with many studies finding a tendency to both overestimate and underestimate risk. Some authors argue that these shortcomings may be due to the model's failure to take into account major surgical issues associated with mortality, such as calcification of the aorta or the complexity of coronary disease, or to the fact that more weight is given to the number than to the type of intervention.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">11</span></a> Moreover, mortality rates reported by the centres participating in the creation of the scale differ widely,<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">12</span></a> hence the concerns raised with regard to the reliability and applicability of the Euroscore II in certain specialties. In the ideal prognostic scale, predicted mortality should correlate with observed mortality (good calibration), and identify potential survivors or non-survivors on the basis of easily measured parameters (good discrimination). For this reason, we performed an external validation to assess the effectiveness of this scale in our patients.</p><p id="par0100" class="elsevierStylePara elsevierViewall">We found that the general characteristics of our population coincide with those described in Spanish studies<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">13</span></a> and in European regions with a similar socioeconomic level.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">14</span></a> Like other authors, we classified surgery by type: coronary surgery, isolated valve surgery, combined CABG plus valve surgery, and other surgery.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">14</span></a> Some studies, however, have classified surgery based on the type of intervention (applying Euroscore II),<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">13</span></a> off-pump or on-pump, or have studied each intervention in isolation.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">15</span></a> The percentage of patients undergoing CABG plus valve surgery was similar to that found in other studies performed in Spain and the Mediterranean region,<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">13</span></a> although the “other surgeries” group is usually smaller, accounting for just 4% of patients.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">16</span></a> “Other surgeries” includes patients requiring neither CABG nor valve surgery, the most prevalent being aortic surgery, intracardiac surgery, resection of intracardiac masses, ventricular aneurysms, surgery for hypertrophic cardiomyopathy, cardiac rupture, ablation of atrial fibrillation and coarctation of the aorta.</p><p id="par0105" class="elsevierStylePara elsevierViewall">General hospital mortality was 6.1%, which is in line with other studies performed in Spain and the Mediterranean region,<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">14,17</span></a> although figures of around 3%, even in high-risk patients and the elderly,<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">18</span></a> have been reported in other countries, such as the UK. Analysing mortality by type of surgery shows that the lowest rate occurs in coronary surgery (1.7%) and the highest in the group of “other surgeries” (24.4%). Mortality among our coronary surgery patients was lower than that reported in the literature, where rates range from 2.1% in the Netherlands to 7.9% in Turkey<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">19</span></a>; mortality among valve surgery patients was standard. Combining CABG with valve surgery increases the complexity of the procedure, and our figures were below those reported in, for example, the US (9.4%).<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">20</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">Both the Euroscore and other mortality prognosis scores (APACHE, SAPS, etc.) effectively separated survivors from non-survivors, with significantly higher numbers in the latter.<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">16,21</span></a> The AUC of Euroscore I and II for the overall population was similar, with good discrimination; SAPS III showed poor discrimination in our patients. Our findings in this regard are almost identical to those of other Spanish studies,<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">13,17</span></a> insofar as discrimination was not improved in the Euroscore II with respect to Euroscore I. Some studies published in Italy, the United Kingdom, Hungary and other countries have observed a slight, though statistically insignificant, improvement in Euroscore II compared to I.<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">8,14,22</span></a> In high-risk patient series, the AUC of both Euroscore I and II is 0.8.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">23</span></a> The poor discrimination of the SAPS III scale is justified, as this scale is not designed specifically for cardiac surgery, and the few studies evaluating it in this setting report an AUC below 0.8.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">24</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">In coronary patients, the AUC of all 3 scales showed good discriminatory power for our series. The best prognosis was obtained with Euroscore I and the worst with SAPS III. Euroscore II showed no improvement over Euroscore I, a finding that contrasts with most published studies, where Euroscore II was found to be slightly, but statistically insignificantly, better than Euroscore I.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">25</span></a> By surgical groups, the Euroscore II showed the poorest discrimination in coronary patients.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">26</span></a> In some population groups, such as octogenarians, Euroscore II shows poor overall discrimination, and is only suitable for use in low-risk patients, as discrimination steadily worsens in moderate and high-risk individuals.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">27</span></a> SAPS III shows acceptable discrimination in coronary patients, a finding echoed by other studies evaluating SAPS II in this patient group.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">19</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">Discrimination in patients with valve surgery patients worsened in the Euroscore, and was unacceptable in SAPS III. This is similar to findings in other studies, some of which reported no improvement in Euroscore II with respect to I,<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">28</span></a> although other authors disagree.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">15</span></a> Some studies have observed that discrimination is better in patients undergoing single rather than multi-valve interventions<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">15</span></a> and in the context of rheumatic valve disease.</p><p id="par0125" class="elsevierStylePara elsevierViewall">We found Euroscore I and II to have excellent discrimination in patients undergoing CABG plus valve surgery, while SAPS III performed adequately in this context. In these patients, Euroscore II showed a striking improvement over the previous version; however, other authors have reported a more modest improvement, albeit with generally acceptable levels that are consistently better in the Euroscore II with respect to I.<a class="elsevierStyleCrossRefs" href="#bib0285"><span class="elsevierStyleSup">27,29</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">We observed good discrimination in all 3 scales in patients without valvular and/or coronary disease, with the best results obtained with Euroscore II, followed by Euroscore I. The SAPS III showed the greater power of discrimination in this group, probably because it is most similar to the general population of critically ill patients.</p><p id="par0135" class="elsevierStylePara elsevierViewall">In the general population, and Euroscore I and SAPS III showed good calibration, while the Euroscore II was not correctly calibrated. Findings in respect of calibration vary widely in the literature. Some articles evaluating Euroscore I calibration include high-risk, elderly patients, and report good results, while in others calibration is found to be poor.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">13,17,29</span></a> Nearly all articles, however, are unanimous in reporting the poor calibration of Euroscore II; the scale tends to underestimate mortality, which is exacerbated in high-risk patients.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">13,14,17,23,29</span></a> Very few studies report good calibration in Euroscore II.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">22</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">Calibration was good in coronary surgery patients using all 3 scales. The vast majority of studies show good calibration in Euroscore II in this group of patients.<a class="elsevierStyleCrossRefs" href="#bib0275"><span class="elsevierStyleSup">25,27,29</span></a> In our study, and in other papers, Euroscore I tends to be superior to Euroscore II in the calibration of this patient population.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">25</span></a> No calibration studies have been performed in SAPS III, although SAPS II has showed good calibration in coronary patients.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">19</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">In patients with valvular disease, Euroscore I showed good calibration, a finding that is echoed in almost all studies performed in both Europe and other regions, such as China, Mexico and Brazil.<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">15,30</span></a> Findings in respect of the calibration of the Euroscore II, however, are less positive, with most, though by no means all, authors reporting the scale to be miscalibrated.<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">15,22,25,30</span></a></p><p id="par0150" class="elsevierStylePara elsevierViewall">We found Euroscore I to be well calibrated and Euroscore II to be exceptionally poorly calibrated in CABG plus valve surgery patients in our series. This is in contrast to the general consensus in the literature, which shows the scale to be well calibrated in these patients.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">22,25</span></a></p><p id="par0155" class="elsevierStylePara elsevierViewall">In patients undergoing surgery other than coronary and valvular, the only calibrated prognostic scale is the Euroscore I, although the Euroscore II is often presented as being well calibrated.</p><p id="par0160" class="elsevierStylePara elsevierViewall">In summary, the Euroscore I showed good calibration in all patient subgroups, the Euroscore II was only well calibrated in coronary patients, and SAPS III was well calibrated in the general population and in CABG plus valve surgery patients.</p><p id="par0165" class="elsevierStylePara elsevierViewall">A comparison of observed vs predicted mortality clearly showed this lack of calibration. In the general population, the Euroscore I and SAPS overestimated mortality and Euroscore II underestimated mortality. This has been confirmed in both Spanish<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">13,17</span></a> and international<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">14,21</span></a> studies, although many showed the Euroscore II prognosis to be closer to observed mortality than the Euroscore I.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">23</span></a> Euroscore I generally overestimated mortality, a miscalibration that became more accentuated as the age and risk level of patients increased.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">16</span></a></p><p id="par0170" class="elsevierStylePara elsevierViewall">Analysing data by surgery subgroup showed that the Euroscore I overestimated mortality in all subgroups, while Euroscore II did so only in coronary patients, and underestimated mortality in the rest. The SAPS III overestimated mortality in all surgery subgroups. In coronary patients, the Euroscore II prognosis was closer to observed mortality,<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">19,21</span></a> although several studies found that it underestimated mortality.<a class="elsevierStyleCrossRefs" href="#bib0275"><span class="elsevierStyleSup">25,26</span></a> The Euroscore II prognosis was closer to observed mortality in our patients with valvular disease,<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">21,25,26</span></a> while in patients undergoing CABG plus valve surgery, the Euroscale I was most accurate. SAPS III was inaccurate in all except for the “other surgeries” group, where it provided the most accurate prognosis. The problem with the SAPS III, apart from its lack of specificity for this group of patients, is that it does not take into account patients not admitted to the postoperative care unit because they died in the operating room, as is often the case in cardiac and coronary surgery.</p><p id="par0175" class="elsevierStylePara elsevierViewall">In conclusion, we observed that the Euroscore II showed adequate discrimination in our patients, although overall calibration was poor, with predicted mortality being lower than observed rates. The Euroscore I, in contrast, showed the best overall discrimination, with good calibration and tendency to overestimate mortality. The SAPS III showed poor discrimination but good calibration, with a tendency to disproportionately increase the risk of mortality.
