Actualización del consenso sobre el abordaje terapéutico del paciente con hiperactividad neurógena del detrusor

Resumen

Introducción y objetivos

En 2018, se publicó un consenso sobre el manejo del paciente con hiperactividad neurógena del detrusor. Este documento pretende actualizar las recomendaciones en relación con el tratamiento recogidas en dicho consenso, con la nueva evidencia disponible, y contribuir a homogeneizar el abordaje de esta disfunción.

Métodos

La metodología utilizada está basada en una revisión sistemática y la técnica de consenso de grupo nominal. El coordinador clínico (CC) y el grupo actualizador del consenso (GAC) definieron las preguntas a actualizar y se llevó a cabo una revisión sistemática para identificar la nueva evidencia. Tras ser evaluada por los expertos, se actualizaron las recomendaciones pertinentes, que fueron consensuadas en una reunión.

Resultados

Se identificaron 3.210 publicaciones y se incluyeron 26 que cumplían los criterios de inclusión. El GAC actualizó 18 recomendaciones sobre el abordaje terapéutico de la hiperactividad neurógena del detrusor (HND). Se alcanzó acuerdo por unanimidad en todas ellas.

Conclusiones

La disponibilidad de nuevos medicamentos, el incremento de la evidencia en torno al uso de la toxina botulínica o los procedimientos de neuromodulación y las alternativas quirúrgicas hacen necesario revisar las recomendaciones.

Abstract

Introduction and objectives

A Consensus document on the management of patients with neurogenic detrusor overactivity (NDO) was published in 2018. The present document aims to update its recommendations regarding treatment considering the new evidence available, and to contribute to the standardization of the management of this disorder.

Methods

The methodology used was based on a systematic review and the Nominal Group Technique. The clinical coordinator (CC) and the Consensus update group (CUG) defined the questions to be updated and carried out a systematic review to identify the new available evidence. After being evaluated by the expert panel, the relevant recommendations were updated and agreed in a consensus meeting.

Results

A total of 3,210 publications were identified and 26 publications that met the inclusion criteria were included. The CUG updated 18 recommendations on the therapeutic approach to NDO. Unanimous consensus was reached on all of them.

Conclusions

Previous recommendations need to be revised due to the availability of new drugs, the increasing evidence on the use of botulinum toxin or neuromodulation procedures, and new surgical options.
